To process your requested tickets in a timely manner, please be prepared to answer the following questions:

If you are a first time caller, please provide the following information:

Main business phone number including area code

Your company name

Mailing address including city and zip code

E-Mail address and/or FAX number for Positive Responses messages

Provide your name and direct phone number if different from the company phone number.

Please advise our DPS if you are a Utility Operator, Contractor, Property Owner or Engineer

You will be asked for the Best Time to reach you.

If you have called Miss Utility previously, our DPS will ask you for your company phone number to begin the process. Your information already exists in our database and our specialist will verify the information for accuracy.

Questions concerning specific information about the proposed excavation site will be asked as follows:

Who will be the Field Contact(the person at the jobsite to contact) and if there is a direct phone number for the field contact, extension or pager?

What type of work you will be doing?

Who is the work being done for?

Will you be using Trenchless Technology?( using horizontal directional drilling equipment or other methods that will tunnel under hard surfaces without opening a trench)

Will you be using explosives or blasting?

What Virginia City or County is the work being done in? Ex.RoanokeCountyor City

Is there a specific place? Ex. Town of Vinton (a place could also be a community within a city).

Do you have the name of the Subdivision and/ or Lot Number?

What is the street address where the work is being done? (The actual house number or numbers if addresses are consecutive – on the same side of the street).

What is the street name? (The actual street name as listed by 911 emergency address system: (1) US Highway ### (2) State Route ### (3) County Road ###).

What are the Cross Streets? (What is the closest intersecting street. It is always best to provide two cross streets).

Is the work is being done in an intersection?

On what side of the property is the work being done?(Standing in the street facing the house is the work taking place on the left, right, front or back. It is best to reference north, south, east or west if at all possible)

Is the area marked in white paint, stakes or flags? (Required by law if a specific address is not given).

Describe the worksite location or area of excavation. (You must provide specific information about the proposed area of excavation).

What additional remarks or instructions would you like to appear of the ticket? (the specialist will ask you for driving instructions to the site if you are unable to provide a specific address)

Do you have a Map Book page number? (this information will assist the locators. Provide the following information: map name, page and grid. Ex. ADC 123K8 or E911 – 43A6)

The DPS will use our QuickMapsoftware to select grids that identify the area of proposed excavation based on the information you have provided. Grids identify the Utility Operators who need to respond to your request.

Our DPSwill provide you with a list of the Utility Operators who will respond to your request.

You will be provided a ticket number once our DPS has completed your notice. You must have this ticket number with you on the jobsite.

Following your call to Miss Utility:

The Positive Response system will automatically respond by E-mail or FAX once all the notified utilities have either marked their lines or have no conflict – no utilities in the area of excavation. If a utility posts a second response, you will receive an additional message. If you have not provided an E-mail address or fax number, you are required to check the system by dialing (800) 552-3120 and follow the instructions.

It is recommended that you request to have the responses to your ticket delivered by E-mail. You will receive a message as soon as all of the utilities have responded and a second E-mail if a utility revises their response.

You may begin excavation once the Positive Response System indicates that all notified utilities have either marked their lines or have no lines present in the vicinity of your excavation.
