Primary Function:
To work as part of a multidisciplinary team to ensure that the mortality review processes are well co-ordinated, integrated and communicated across all maternity stakeholders.
The role will involve being an agent of the PMMRC. The role will also facilitate and enable communication and co-ordination with parents/family/whanau as well as support linkages with external agencies.
Key Accountabilities

Demonstrates efficient management of local administrative requirements for PMMRC / •  All Perinatal and Maternal deaths are notified to the Perinatal Midwife Specialist
•  Manages and maintains the local PMMRC database
•  Work with SMO Local Coordinators to ensure the following occur regularly
PERINATAL
•  All rapid reporting form(s) have been completed and data submitted to PMMRC following Perinatal death
•  All reports are followed up e.g. post mortem and placental histology in a timely manner
•  Regular classification meetings are facilitated by the local coordinator and all classification forms are completed using PSANZ classifications and submitted with additional information to PMMRC
MATERNAL
•  Facilitates the submission of data on the PMMRC Maternal Mortality Form following a maternal death
•  Supports and attends the DHB mortality review
•  Facilitates the collection of additional data as required by the PMMRC
Supports, manages and facilitates local PMMRC meetings / ·  Monthly meetings are adequately prepared with notification to stakeholders and presenters
·  Appropriate follow up post meeting including recommendations
Effectively manages local
administrative requirements
for AMOSS / 100% of required monthly data collected and reported to AMOSS
Maintain strong links as DHB
Liaison for Stillborn and Newborn Death Support Group (SANDS) / Demonstrates effective networking and communication to SANDS in terms of developing and making available resources for women and families/whanau. Provides a visible link between the community and DHB.
Demonstrates an ability to lead quality activities to improve clinical standards / •  Maintains confidentiality in accordance with the dual role of agent of the National Review Committee and employee of the DHB
•  Supports and implements local and national initiatives to improve care to bereaved parents/whanau
•  Recognised clinical resource for root cause analysis investigation
•  Develops clinical guidelines, policy and clinical pathways to reflect changes in legislation related to Perinatal and Maternal death or recommendations to improve clinical standards.
Effectively supports and teaches other health professionals in the care of women and babies / •  Provides leadership in the care of women and their babies including care of deceased babies. Is a resource for other staff.

8  Education & Qualifications

/ Essential / Desired
Professional / Experienced registered midwife with 5 years community and hospital sector experience / Tertiary qualification in Health and completed or working towards postgraduate qualifications
Must be a clinical expert in midwifery and have the ability to provide direction for and evaluate midwifery care
Experience/Knowledge / Excellent interpersonal skills and the ability to establish and build upon working relationships effectively in order to successfully achieve
outcomes / Experience in sentinel event or
root cause analysis processes
and report writing
Balanced and informed approaches to problem solving / Project management experience
A pro-active goal orientated and focussed approach to the tasks and responsibilities associated with the role. / §
