*You are responsible for the safety of your child, you must maintain constant sight of your child and intervene in any unsafe behaviors. Aspire Girls of the Puget Sound Parent Volunteers cannot provide childcare. By signing you are agreeing to these terms____(initial)
* All adults are required to behave in a safe manner, use appropriate language, help to provide a safe and calm environment for the children at all times. Any inappropriate or dangerous behavior will result in immediate dismal from the group. By signing you are agreeing to these terms____(initial)
*To participate you must refrain from smoking, By signing you are agreeing to these terms___(initial)
*To participate you must not carry or display weapons. By signing you are agreeing to these terms___(initial)
*Aspire Girls is an official association of Washington Autism Alliance and Advocacy. As such the WAAA Staff, Jess Silvernail, and Rachelle Alston, Executive Assistant and Program Coordinator, as well as our certified BCBA volunteer, Jaymi Shull must report any criminal or abusive activity to the appropriate authorities immediately. By signing you are agreeing to these terms____(initial)
You may not participate in Aspire Girls if you have ever been or are in the future
(a) Convicted of any crime against children or other persons;
(b) Convicted of crimes relating to financial exploitation of a vulnerable adult;
(c) Convicted of crimes related to drugs;
(d) Found in any dependency action under RCW 13.34.040 to have assaulted or exploited any minor or to have physically abused any minor;
(e) Found by a court in a domestic relations proceeding under Title 26 RCW to have abused or exploited any minor or to have physically abused any minor;
(f) Found in any disciplinary board final decision to have abused or exploited any minor or developmentally disabled person
(g) Found by a court in a protection proceeding under chapter 74.34 RCW, to have abused or financially exploited a vulnerable adult. I also recognize that Aspire Girls of the Puget Sound in association with Washington Autism Alliance and Advocacy will make a Request for Criminal Background History as ordained in the Child/Adult Abuse Information Act: RCW 43.43.830 through 43.43.845
By signing you are agreeing that you have not been convicted of any acts outlined in A through G and agree to the terms that you cannot participate in Aspire Girls if you become convicted at a future date ____(initial)
*Participation in Aspire Girls is conditional on the results of your background check. By signing you are agreeing that you understand that if any criminal activity is found in your background check you will not be allowed to participate ____(intial)
*Washington Autism Alliance and Advocacy and Aspire Girls of the Puget Sound reserve the right to remove any member at any time for any reason. By signing you are agreeing to these terms ____(initial)
*You have the right to have any private information shared with the Parent Volunteers to be kept confidential, unless it is criminal or abusive in nature. By signing you are agreeing to these terms____(initial)
*You may not share or publish any information relating to the personal information of members or the location of meetings of Aspire Girls of the Puget Sound without the expressed permission of the WAAA Staff, Jess Silvernail, Rachelle Alston. By signing you are agreeing to these terms____(initial)
