Site-license purchases require: 1) a purchase order identifying a single individual as the primary point of contact; and 2) A signed copy of the “Vox Proxy Application for Educational License”. A single CD will be shipped. The license permits duplication of that CD for the number of licenses purchased. All support is provided to a single, designated individual only.

Special for K-12 site-license purchasers only: individually-licensed copies for the school itself, students or educators may be purchased at the fixed price of $39.95 each for Vox Proxy standard version and $54.95 for Vox Proxy with CD Prep.

All purchasers of single user licenses must complete and submit the Application for Educational License. These purchase requests must be collected and submitted by the school that purchased the site license.

Vox Proxy™ Educational Pricing Qualifications

Only qualified students, faculty members, staff members and accredited institutions are eligible to purchase Vox Proxy software licenses at reduced Educational prices.

INDIVIDUALS:

· K-12 students currently enrolled at an accredited public or private primary or secondary school providing full-time instruction.

· Full or part time students currently enrolled at accredited public or private university or college, including community, junior or vocational college that grants degrees.

· Full or part time faculty and staff employed by institutions as defined below

All individuals must fax or e-mail a completed APPLICATION FOR EDUCATIONAL LICENSE along with a copy of your dated school photo ID card validating current status. If your ID isn't dated for the current year, we will also need one of the following:

· Another dated school ID (library card, etc) that has your school's name on it.

· Letter from the school on official letterhead stating that you are currently affiliated with

the school.

· Copy of faculty or staff pay check stubs (faculty or staff only).

· Student course schedule or tuition bill with valid date and printed with the school's logo

or seal.

For K-12 students only: if a dated school photo ID card is not available, a letter on official school letterhead from a teacher at the accredited school verifying the student's eligibility will be accepted.

INSTITUTIONS:

· Preschools with early childhood programs that provide educational services.

· K-12 institutions defined as accredited public or private primary or secondary school providing

full-time instruction.

· Higher education institutions defined as accredited public or private university or college,

including community, junior or vocational college, which grant degrees .

All institutions must fax or e-mail a completed APPLICATION FOR EDUCATIONAL LICENSE

along with a valid, official purchase order issued by the institution or current credit card information with the proper name, billing address, and 'ship to' or delivery address of the educational institution.

EDUCATIONAL LICENSE LIMITED USE:

Use of Vox Proxy software for academic and educational purposes means use by you for purposes directly related to teaching, training, and degree-granting programs. If you are a student, this Software can only be used in conjunction with your coursework and for projects supported by your school. Educational License users cannot use Vox Proxy software for any commercial purpose, including but not limited to, commercial processing, paid-for consulting, processing the work of third parties, developing software for sale or license.

Section 1: User Qualifications

Section 2: Terms and Conditions of Use

The undersigned acknowledges that the user of the Software is bound by the terms and conditions of the SOFTWARE LICENSE AGREEMENT FOR Vox Proxy software set forth in the click license that appears upon installation of the Software. If the undersigned has applied for a site license, the undersigned acknowledges that the Software may only be accessed and used at the Designated Site or otherwise in accordance with the terms and conditions of the SOFTWARE LICENSE AGREEMENT and that access is limited to the number of licensed users as specified above. The undersigned further acknowledges that the Software is licensed for educational purposes only and that any commercial use of the Software will immediately terminate the undersigned’s right to use the Software.

Section 3: Entire Agreement

The undersigned acknowledges that the undersigned had read this agreement, understands it and agrees to be bound by its terms and conditions. The undersigned further agrees that this agreement, together with the SOFTWARE LICENSE AGREEMENT FOR Vox Proxy (the “click license”), is the complete and exclusive statement of the agreement between the parties which supersedes any proposal or prior agreement, oral or written, and any other communications between the parties relating to the subject matter of this agreement. All terms and conditions of the SOFTWARE LICENSE AGREEMENT FOR Vox Proxy remain unchanged and are applicable to this agreement.
