Job purpose
Providing a social prescribing service to help reduce social isolation and increase the well-being of eligible individuals in the Greater Fishponds area by;
- Providing one to one support
- Providing opportunitiesfor volunteering, and
- Facilitating group events.
Specific tasks
- One-to-one casework support
- Receive and respond to new referrals in a timely fashion
- Provide an assessment and signposting service for the clients which responds to their individual needs and addresses access requirements
- Provide empowering support to clients to access community services in such a way that enables access and overcomes barriers
- Undertake W2W evaluation systems to measure the journey of the client
- Research local opportunities to enhance service users’ well-being
- Maintain up to date case work notes on the casework management system, collect and collate relevant data in accordance with the Data Protection Act (DPA).
- Provide opportunitiesfor volunteering
- Recruit, train and support volunteers for the project, including identifying potential service user volunteers
- Identify volunteers to match with volunteering opportunities within W2W. For instance, one-to-one casework, administrative roles, Advisory Group roles, social group support.
- Offer volunteers support suited to the role(s) they have taken on. For instance, supervise volunteer’s casework notes, provide a safe-working environment, supply data for Advisory Group papers and suitable administrative tasks for administrative volunteers.
- Collect, collate and maintain records about volunteers in accordance with the DPA.
- Facilitate group events
- Arrange the venue, date and time for a weekly social group.
- Provide support to enable service users to attend and participate as fully as they wish.
- Provide facilitation for the group to enable and empower participation, taking into account any access requirements.
- Collect, collate and share data about the group for monitoring and development purposes.
- Run awareness raising sessions about the service
- Provide a drop-in facility at GP surgeries or other community venues
- Other tasks
- Find new and innovative ways of enabling people who are referred for social prescribing to support each other and contribute to the development and improvement of the project.
- Provide the project in line with the TCF chosen model of social prescribing.
- Obtain and pass on to Well Aware, information on the range of local services available and encourage local residents and groups to use Well Aware.
- Collect and collate monitoring information and feedback on the project, and maintain the social prescribing spreadsheets
- Work with other members of the social prescribing project to effectively raise the profile of the project with all stakeholders and increase referral rates.
- Monitor and review the case management system and evaluation process with the aim to inform wider learning across Bristol social prescribing services.
- Write a quarterly newsletter from the project which is sent out to all service users.

- Act at all times as a positive and proactive ambassador for The Care Forum and all its services.

Experience
1 / Paid or voluntary work in the health and social care sector (voluntary sector desirable)
2 / Work with vulnerable adults such as people who have a mental health problem, physical impairment or learning difficulty
3 / (Experience of community development and working with and supporting volunteers)
Skills
4 / IT skills including word processing, e-mail, spreadsheets and use of databases
5 / A high level of organisational, planning and prioritisation skills including an ability to work to deadlines and use own initiative

Excellent interpersonal and communication skills, written, verbal and listening
7 /Effective skills to plan and run awareness raising sessions about the service to a wide audience
8 / Ability to work in an empowering and empathetic way with clients who at times may be emotionally distressedKnowledge
9 /Understanding of the support needs of an individual involved in seeking or using primary care services
