B. This form should be filled in electronically by the DPO and sent to the email address in Note A. Allquestions and boxes must be filled in with text, figures or ‘not applicable’.
C. If an agent or other unauthorised person is signingthe form on behalf of the DPO, you must fillin a DPS8 form. / D. This form must be filled in and lodged with RPA before negotiations with a raw milk processor/collector. Complete a separate form for each negotiation.
E. This form may be filled in as a hard copy and sent to the address at the bottom of the page.

An indication of which other processors/collectors the DPO might supply in this geographic area.
An indication of the proportion of the market covered by the parties to the negotiation.

· The producers concerned by the negotiation are not members of any other producer organisation which also negotiates such contracts on their behalf (unless approved by RPA in accordance with Article 149 of Regulation 1308/2013).
· The milk subject to the negotiation is not the subject of negotiation by another DPO or covered by an obligation to deliver arising from a producer’s membership of a cooperative.
