INFORMATION TECHNOLOGY (IT)QUESTIONNAIRE

Vendors shall submitthe answers to thisAttachment Iin Folder 3of their response, answering all questions in narrative format.The answers provided by Vendors will be evaluated as described in Section 3.7, Evaluation Process.Vendors are requested to limit the response to this Questionnaire to a total of 30 pages (including any attachments, brochures, or other documents attached to or embedded in the response).Answers must be specific to the System described in Attachment H,General Questionnaire and priced in Attachment J, Price Sheet.

Capitalized terms (such as System, Vendor, Internal User, etc.) shall have the meanings provided in Section 1.3 of the ITN.

Section I – General Questions (Up to 7points)
