Practice Management User Responsibility

This policy outlines the responsibilities of users of University Eye Institute (UEI) computing equipment and the Practice management System. It is the responsibility of each UEI computer user to avoid the possibility of misuse, abuse, or security violations related to computer use. Each user is responsible for becoming familiar and complying with guidelines, policies, and procedures relating to university computing equipment and systems. This policy is in compliance with UH System Administrative Memorandum 07.A.03, University MAPP10.03, University of Houston Information Security Manual, Computing Facilities User Guidelines and other applicable state and federal requirements.

PROCEDURE

  1. Confidential passwords shall be assigned to provide access to Practice Management System and shall be changed yearly. Users with last names beginning A thru M will change passwords during month of February with all changes completed by February 28th. Users with last names beginning with N thru Z will change passwords in August, with all changes completed by August 31st. Users shall also receive a copy of the UHCO user policy and guidelines during their password change.
  2. Users of computers and computing systems must respect the privacy of other users. For example, users shall not seek or reveal information on, obtain copies of, or modify files, tapes or passwords belonging to other users, nor may the user misrepresent others.
  3. Users must respect the intended university business or academic purpose for which access to computing resources is granted. Examples of inappropriate use of university computing resources include, but are not limited, use for personal or corporate profit or for the production of any output that is unrelated to the objectives of UEI.
  4. In accordance with established university practices and numerous state and federal laws regarding computer violations, a user found to be abusing or misusing university computer resources may be subject to disciplinary action up to and including expulsion from the university or termination of employment, and/or to legal action.

It is your responsibility to read and abide by the established policy and procedures, attached to this form.

This policy requires users to change their password once a year. Your password should contain a combination of six letters and numbers. Write your new password in the space provided, sign and return to Ed Waller by ______.
