Job Purpose:
  1. To assist in providing a professional Occupational Therapy Service to all customers of the Social Services Department.

Principle duties and responsibilities:
  1. To undertake less complex assessments under the supervision of a qualified Occupational Therapist for people who are currently living in the community or are returning to living in the community.
  2. To follow up the assessment/recommendations made by the senior, or other Occupational Therapists and provide any further progress information/reports.
  3. To assist in prioritising referrals utilising the agreed prioritisation procedures.
  4. To instruct and advise customers, carers and colleagues in the use of aids and equipment.
  5. To undertake assessments for minor fittings and minor adaptations.
  6. To undertake assessment for major adaptations under guidance from a qualified Occupational therapist
  7. To liaise with, and work in partnership with colleagues of the County Council and from other agencies.
  8. To ensure that individuals and their carers have the information made available to them in an appropriate format, which enables them to make informed choices.
  9. To ensure that the service is maintained at the highest standard and that information systems including CareFirst and customer files are maintained and up to date.

Person specification – Qualifications, Knowledge, Experience & Skills: / Essential / Desirable
  1. Practical knowledge of Social Services and Wellbeing Act and related legislation.
  2. Educated to GCSE standard with a minimum of 5 GCSEs including English and Maths.
  3. Ideally have NVQ Level 3/ HNC Community Care.
  4. Must have experience of working in a caring profession
  5. A thorough working knowledge of the Microsoft suite of software and good IT skills, ideally with CareFirst experience.
  6. Ability to work on own initiative with support from the Occupational Therapy staff.
  7. Ability to work positively with other agencies.
  8. Clear and concise written and verbal communication skills.
  9. Good organisation skills and the ability to work under pressure.
  10. Understanding of the need for confidentiality.
  11. Knowledge of the Welsh language would be desirable.
  12. Must possess a valid driving licence and car available for work.
