It is with great pleasure that we are introducing the Elective Catalog. The Elective Catalog has a significant purpose. It is designed to expose residents and interested applicants to the wide variety of rich psychiatric experiences available in our program. The availability of this information in a single resource should facilitate residents in selecting electives during their residency training, allowing residents to pursue a rich and diverse educational experience.

Please note that residents are not limited to the plethora of electives described in this catalog. Residents have the option of “making their own elective” based on individual preference and supervisor/mentor availability in that area of interest. In fact, residents have the option of contacting providers, if they wish to work with that provider in their area of expertise. However, this should be done in close collaboration with leaders in our residency program.

Chemical Dependence

  4. Goals/Objective: The study of addiction psychiatry in patients who are undergoing rehabilitation. Residents are asked to see patients with acute psychiatric needs.
  5. PGY3: half day per week
  6. Experience: Locked rehabilitation unit for patients with addiction issues. Residents find it similar to addiction psychiatry in the inpatient setting. The supervisor is not on site, allowing for autonomy. No exposure with suboxone and other addiction drugs

Child Psychiatry

  6. Experience: full responsibilities of PGY2 resident, very close interaction with attending on unit. Very educational experience.

  7. Experience: Case Counseling Center is a very nice setting to learn. It is very close to UH. Software is nice. Dr. Kimmel is a great supervisor. Residents have scheduled meetings with psychologists to discuss therapeutic aspects of psychiatric treatment.

It also allows residents to work with providers on a multidisciplinary level (case-managers, NP, visiting nurses) and in multiple settings (Magnolia Clubhouse, CFC's access clinic, and MHS's homeless outreach). Another advantage is that you get time with Dr. Runnels who is interesting and challenging

  2. Goal/Objective: A deeper understanding of the psychopharmacological management and treatment of individuals suffering from treatment-resistant schizophrenia, on clozapine.
  3. Day/Time: Clinic meets on Thursdays for the entire day, PGY2-PGY4
  4. Experience: Reportedly, the clinic carries around 20 patients. Patients come in to group sessions in the morning followed by individual medication management. Resident works closely with other members of treatment team.
  1. VNA elective:
  2. Location: Residents would meet with VNA nurses and work with them by going directly in the homes of clients referred to the VNA.

  5. To understand the challenges and benefits of seeing patients in non-traditional settings
  6. To enhance understanding of working with a multidisciplinary team in a non-traditional setting
  7. To develop understanding of finding resources for patients who do not have traditional routes of access to insurance or treatment
  8. To broaden understanding of how larger systems and structures - governmental, private non-profit, and private for-profit - impact the lives, symptoms and treatment of individuals with mental health issues
  9. To gain exposure to individuals whose symptoms often fall outside traditional diagnostic categories as listed in the DSM
  10. To enhance diagnostic formulation and understanding, particularly as to how social history impacts phenomenology
  11. Day/Time: Typically Thursday mornings, but that can be augmented as needed, 4 hours/week for 6-12 months. This elective is designed for PGY-4 residents as they can practice the elective longitudinally. Please contact Dr. Runnels if you have an interest in exposure to this as a PGY-2 resident.
  12. Brief Description:The Visiting Nurses Association has a long tradition of providing services in home to patients who struggle to access healthcare in more traditional settings, including mental health services. More recently, the VNA has worked with the county and state to provide a 'bridge' for un and underinsured patients who have visited the ER, and were not deemed appropriate for admission, but need more urgent care during the period of time between ER visit and first appointment with an outpatient provider. Residents will spend one morning a week going out with a psychiatric nurse to provide evaluation and treatment to these patients.
  1. Interested residents also have the option of completing a “Homeless Mentally Ill Outreach” Elective. This elective is usually combined with the VNA elective. For example, residents usually do the VNA elective during the day and the “Homeless Mentally Ill outreach” elective in the evening. Similar goals/objectives and time requirements as the VNA elective. Consult-Liaison Psychiatry

  6. To master diagnosis/treatment and understanding of the biopsychosocial formulation of somatically ill patients with significant psychiatric comorbidity
  7. To obtain a deeper understanding of neuropsychiatric cases (stroke, epilepsy, movement disorders, different forms of dementia) where a high comorbidity is observed between neurologic and psychiatric symptoms
  8. To obtain mastery in the completion of transplant evaluations
  9. To determine suitability of psychotherapy intervention in the setting of complex neuropsychiatric cases
  10. Day/Time requirements: Tuesdays 13:00 – 17:00
  11. Additional information: CL psychiatrists work in a multi-specialty and interdisciplinary environment in clinical cases characterized by high comorbidity between psychiatric and physical illness. Hence, CL psychiatrists are valuable in the delivery of comprehensive medical care in integrated, outcome-oriented systems. As skilled diagnosticians, CL psychiatrists are expected to decipher biopsychosocial formulations that contribute to effective psychotherapeutic and psychopharmacological.

Developmental Disability

  1. Community psychiatry in ID/DD (developmental disabilities)

  4. Goal: gain clinical exposure in working with dual diagnosis patients (ID+psych illness); this includes observing and participating in initial evaluations, follow ups, case formulations, treatment plans. Also, visits various outpatient sites in the community including Walker, St. John Westshore office, and Youngstown developmental center. Met with Dr. Ruedrich's colleagues from the county board of ID.
  5. Experience. Residents have enjoyed working with Dr. Ruedrich. He spends a lot of time with each patient and provider, is eager to teach, provides several resources (staff contact info, old research papers, books), has a very approachable demeanor, and the elective is extremely flexible. The resident creates his/her own goals/objectives.
  6. PGY2: 4 week elective. Mon are full days at St. John, Tuesday & Wednesday are afternoons at Walker, one Thurs was full day at the Youngstown developmental center, and Fri full day at Walker.

Eating Disorders

Ethics

  1. Ethics Elective for Psychiatry Residents:

Goals/Objective of elective: This elective is intended to give psychiatry residents an opportunity to experience and learn about ethical issues that are relevant to psychiatry and general medical care. During this elective, residents would be involved with the Hospital’s Ethics Consultation Service, and would be expected to attend the various ethics committee/council meetings (UH/Case Hospital Ethics Committee, MacDonald Hospital for Women’s Ethics Committee, RB&C’s Ethics Committee and Psychiatry/Richmond Ethics Council meetings). Also, a core set of readings about medical ethics and psychiatric ethics will be expected during this rotation, and opportunity to meet with Dr. Stagno, Dr. Daly, or others from the ethics consult service to discuss them should be arranged.
  5. Time: 2 week or 4 week elective experience. The resident may choose to focus on a particular area related to medical ethics for in-depth literature review, a scholarly paper, or other project.
  6. Dr. Daly must be contacted in advance of scheduling this elective because of the number of trainees from various programs that spend time on the Ethics Consult service.

Forensic Psychiatry

Neuropsychiatry/Geriatric Psychiatry

  4. Goal/Objective of elective: Exposure to various psychological tests employed in diagnosing psychiatric diagnoses
  5. PGY4: part-time elective, only certain days of week
  6. Experience: useful experience to observe patients undergoing neuropsychological testing and obtain a deeper understanding of such testing. It is encouraged that residents plan this elective in advance with Dr. Poreh as he tends to see patients on certain days of the week only
  1. Outpatient Geriatric Psychiatry

  4. Goals/Objectives: The study of geriatric psychiatry in the outpatient setting
  5. PGY3: half day per week
  6. Experience: exposure to mental health issues affecting the geriatric population in the outpatient setting. Closely supervised by Dr. Dines. Recommended for those interested in geriatric psychiatry
