Copeman Healthcare Centre is seeking an enthusiastic Family Nurse Practitioner for their Calgary clinic. The position currently requires a Permanent Full-Time Equivalent.

The focus of this position is a Family Nurse Practitioner Model. The diversity of the position requires a Nurse Practitioner more senior in their practice to provide expertise in clinical areas working with individuals from all age groups.

Responsibilities include, but not limited to:

ESSENTIAL FUNCTIONS AND RESPONSIBILITES

  • Work collaboratively with other team members to manage the needs of the patient roster of the Family Health Team (FHT).
  • Individual patient assessment and treatment - health assessment, immunization, ordering laboratory-screening tests, diagnosis, treating illness and follow up, health promotion and disease prevention counselling.
  • Conduct full patient assessment and prepare a plan for intervention and/or for referral to FHT programs as appropriate.
  • Identify, analyze and provide appropriate treatment and/or referral for patient's physical and psychosocial health care problems.
  • Co-ordinate appropriate diagnostic tests and procedures as per FNP scope of practice, and/or in consultation with family physicians or in accordance with medical directives.
  • Implement various goal-oriented, time-limited intervention plans, including education in collaboration with patient and/or family and inter-disciplinary team.
  • May be required to work at more than one Copeman FHT site.
  • Participate as a clinical team member in the development and delivery of targeted programs including but not limited to palliative care, cancer care, case management, chronic disease, mental health and addictions, reproductive and child health.
  • Collection of data as required for statistical/reporting purposes.
  • Identify gaps and needed modifications in services.
  • Facilitate referrals to other services as well as communication and networking between the primary care practice and hospital or home care.
  • Represent the mission, values and philosophy of the FHT in the community, including networks, meetings, or presentations, as requested.
  • Assist in training and supervision of students and/or volunteers as requested by the Executive Director and/or Lead Physician.

  • Become familiar with all of our products and services by reviewing product literature, observation, note taking and asking questions
  • Provide clients/patients with undivided attention

  • At the direction of management, perform duties for affiliated companies
  • Other duties as assigned

Qualifications

  • A graduate degree as Family or Primary Health Care Nurse Practitioner.
  • Must be a member in good standing with the CARNA.
  • Additional training or credentialing is considered an asset (Certified Geriatric NP, Certified Diabetes Educator, Certified Asthma Educator, Certified Respiratory Educator, TEACH certification for smoking cessation, Psychiatry Certificate Program, Cardiovascular Certificate Program).
  • Basic CPR certification. ACLS certification considered an asset.
  • Current experience in chronic disease management preferred.
  • Knowledge and demonstrated nursing ability in community health and public health sciences and chronic disease management.
  • Knowledge of client-centered philosophy, change management, basic principles and practices of community service delivery, short-term treatment options and chronic disease management models.
  • Knowledge of community resources connected to the provision of health promotion and illness prevention related services.
  • Knowledge in Travel Health.
  • Knowledge and experience with pediatric populations.
  • Demonstrated skills in assessment, program planning, implementation and evaluation.
  • Advanced Clinical nursing practices within the scope of the FNP.
  • Exceptional organizational and personal skills
  • A team player capable of interacting professionally with staff with a positive “just do it” attitude
  • Accountability and recognition of its importance in the healthcare environment
  • Excellent written and communication skills
  • Excellent organizational and time management skills with the ability to take initiative and work with limited supervision but within the guidelines of the position
