REQUEST FOR PROPOSALS

Purpose:
DCEA Affordable Housing is seeking qualified firms to conduct an inspection and submit proposals for mold remediation and repairs to 26 units at above address.
The on‐site manager will arrange access to the units.
INSTRUCTIONS AND SCHEDULE FOR SUBMITTAL OF PROPOSALS
All Proposals shall be signed and sealed by a duly authorized representative of the Company. The name
and mailing address of the individual executing the proposal must be provided.
DCEA Affordable Housing shall not be liable for any pre‐agreement expenses incurred by any company in relation to the preparation or submittal of Proposals. Pre‐agreement expenses include, but are not limited to, expenses by company in: preparing a Proposal or related information in response to request for proposal, or on any matter related to this request for proposal including but not limited to, travel or presentations. Additionally, DCEA Affordable Housing shall not be liable for expenses incurred as a result of DCEA Affordable Housing rejection of any Proposals made in response to this request for proposal.
All proposals shall be based on the criteria listed in the attachment of each effected unit that was entered into Exacta Mate software from the initial inspection and in the opinion of each contractor may or may not be needed.
Although we will require a quote for each line item based on the initial inspection please include your determination as to the need of each line item on all units.
Any discrepancies in work other than in the attachments may be submitted in a separate report and proposal.

There will be a mandatory inspection of the units on 05/02/2017
Beginning at 8:00 A.M.
Proposals are to be submitted on or before 05/04/2017 no later than 3:30pm

Contractor awarded must provide proof of 1 million each occurrence liability insurance.
All bidders are welcome to be present.
DCEA Affordable Housing reserves the right to reject any and all proposals and to waive information and
minor irregularities in any proposal received.
EVALUATION PROCESS AND SELECTION CRITERIA
DCEA Affordable Housing will conduct the selection process and is the final decision‐maker regarding this selection, and it reserves the right to reject any or all responses or to terminate development
negotiations at any time.
DCEA Affordable Housing reserves the right to request clarification or additional
information from individual respondents.
