PUPPY QUESTIONAIRE

The puppy’s welfare is our foremost consideration in placement of a puppy. We want to ensure that the “right” puppy is placed in the “right” home, based upon the puppy’s attributes, including personality, as well as the potential owner’s requirements and abilities. We realize that some of the questions below may seem intrusive – it is not our intent to pry – but, to ensure that all three of us (breeders, owners and puppy) will be appropriately matched. This puppy is a lifetime commitment, not only for us, but for the prospective owner. Consequently, we need the ability to cooperatively work together, especially in the unfortunate event that any issues arise with the puppy. Accordingly, please answer the following questions as completely as you can to aid us in this matching process.
