NOMINATION FOR FELLOW OF THE AMERICAN CERAMIC SOCIETY
Qualifications of a Fellow:
Fellows shall be persons of good reputation who have reached their 35th birthday and who shall have been members of the Society at least five years continuously at the established nomination deadline date when nominated. They shall prove qualified for elevation to the grade of Fellow by reason of outstanding contributions to the ceramic arts or sciences; through broad and productive scholarship in ceramic science and technology, by conspicuous achievement in ceramic industry or by outstanding service to the Society.
Nomination Procedures:
* This form must be signed by seven sponsors, who must be members of The American Ceramic Society at the time of nomination. Faxed, scanned, photocopied, and electronic signatures are all acceptable in lieu of an original signature. Signatures do not need to be on one form, but may instead be sent to the Society on individual forms, as long as a total of seven sponsors' signatures are received.
* The nomination shall be accompanied by a single page letter from the first sponsor setting forth the nominee's qualifications. In addition, the first sponsor may submit no more than three additional single-sided sheets to provide supporting information. More than 3 pages of supporting information will result in the nomination being returned to the first sponsor.
* In addition to the letter and supporting information from the first sponsor, single-page endorsement letters from other sponsors may accompany the nomination but are not required.
* Once received, a nomination may be considered by no more than four consecutive Panels of Fellows. The nominee must remain an active member throughout this time.

1. PROFESSIONAL BACKGROUND: Positions held prior to present position, including dates, nature, and extent of duties in each position; colleges or professional schools, with years of attendance and degrees; honorary degrees, awards, distinctions, professional registration.
2. PROFESSIONAL AND CORPORATE ACHIEVEMENTS: Outstanding work, including unpublished research and corporate achievements, in the field of ceramics with which the nominee has been associated in a responsible capacity and his/her relation to this work. If nomination is based principally on practical achievements, responsible charge of important ceramic work for at least three years is required.
