Yes. Hello Mrs. Jones, this is ______from Dr. ______’s office. He asked me to give you a call and check with you on your health. Do you mind if I ask you a few questions? It will only take a minute?

Questions:

1.  How have you felt since your last visit to ______(clinic name)? Has your pain been reduced?

2.  Have you had any problems or reoccurrence of pain since your last visit to ______(clinic name)?

3.  Did you feel that your visits to ______(clinic name) successful?

4.  Was there any particular reason for not returning to ______(clinic name) for regular check-ups?

Thank you for your answers. I will pass them on to Dr. Smith. Dr. Smith also asked me to invite you to visit our office and receive a complimentary health check-up with him since you haven’t been in for a while. He has also authorized a free fifteen minute massage for you after your check-up. Could I schedule you for your visit now?
