This Confidential form is used to disclose a physical or biological entity that is useful in research. as required under Florida State University (FSU) Policies (Explained in Policy on Patents and University-Sponsored Educational Materials. please respond to each Question even if the answer is "none" or "not applicable". FSURF will share any royalty income derived from the license of the material with the persons listed below according to its standard policies (http://facultyhandbook.fsu.edu/Section-6-Policies-and-Procedures#Patents) as may be updated from time to time.
