The Captain of each team will be held responsible for all team requirements being met. The undersigned Captain agrees to and understands as follows:
1. The Captain personally guarantees all amounts that their team is obligated to pay (the team fee for the season or any other specifically agreed upon service) to Hockey Detroit LLC. The Captain’s responsibility for payment is not affected by the failure of team members from paying their allocable portion of the team fees.
2. All participants must be registered with USA Hockey and pay the membership fee.
3. All participants must have signed and returned their copy of the Release of Liability Waiver provided by the league.
4. All players must have uniform jerseys with individually identifying numbers.
5. Your team is subject to a forfeit, as well as other consequences, if a player participates that has not registered with the league and USA Hockey.
6. This is a no checking/no fighting league. Teams and players are to follow these and all other league Rules.
7. I understand that it my responsibility to follow-up with the league (Kathy at ) to make sure that my official USA Hockey team roster is complete and accurate.
