NRCS or KDFWR Farm Bill Biologist (FBB) shall complete all sections down to and including the Planned Practice(s) section. Once completed, this form can be emailed or otherwise delivered to the applicable KDF Chief Forester (KDF)and KDFWR Private Lands Biologist (PBL), when needed, to request technical assistance. The Chief Forester or acting Chief Forester will forward the TAR to the appropriate KDF Service Forester (SF) for processing. The KDF SF with thePLB, when needed, will complete the remaining sections starting with the Practice Units Table down to and including signatures on the “Needs Determination” section. Once complete, the KDFSF will mail or handdeliver a copy of the completed form and “Practice Prescription Plan” to the NRCS or FBBcontact. The KDF SF will keep the original and use it for practice completion certification. Once the KDF SF and the PLB, if needed, have completed the applicable ‘Units Applied”section and signed the “Completion Certification”, the TAR will be mailed or hand delivered to the NRCS or FBB contact. NRCS/FBB shall make no practice certifications or payments until applicable Units Applied sections are completed and the Completion Certification is signed by KDF SF and the PLB if needed.

Needs Determination Section
KDFWR signature in addition to KDF’s is required for planning a FSP2, Upland Wildlife Habitat Management (645) edge feathering or Restoration and Management of Rare or Declining Habitat (643) woody vegetation removal from remnant grassland practice.
Needs Determination: We have determined the practice(s) is needed and practical and that the recommendation is according to the applicable NRCS practice standard and program guidance. Establishment details are included on the attached practice prescription plan(s) for your use in contract and job sheet development.

Completion Certification Section
Either KDF or KDFWR signature can be used for certification of Tree and Shrub Establishment (612), Riparian Buffer (391), Upland Wildlife Habitat Management (645) edge feathering or Restoration and Management of Rare and Declining Habitats (643) practices. Only one signature is required. Neither KDF or KDFWR signature is required for 612 or 391 tree plantings completed under the CRP/CREP since producers self certify these practices for the CRP/CREP.
Only KDF signature is required for any Forest Stand Improvement (666) practice certification.
Completion Certification: To the best of my knowledge, the practice(s) listed above have been completed according to plans and specifications. See the table above for units applied.
