DO NOT MAIL ANY FORMS TO TARTAN HIGH SCHOOL. THEY MUST BE HAND DELIVERED TO THE ACTIVITIES OFFICE FOR VERIFICATION.

If an athlete needs an athletic lock, it can be purchased through the Activities Office. It will be purchased at a time different than at sign up. The cost will be $5.00. The athlete will keep it year to year.

Welcome to Tartan Athletics! The following information should help to sign up your athlete this year.

1. The listing of sports offered, fees and beginning dates are listed on the other side.

2. To sign up for a sport, the following needs to be completed and turned into the Activities Office:
*1. MSHSL Eligibility Brochure (both sides)
*2. Emergency form
*3 Sports Qualifying Physical Examination Clearance Form**
**Physicals:
1. A current physical must be on file in the Activities Office.

The physical will expire three years from the date the physical was given.
2. For the 2013-14 season, 2010-11 physicals will expire. Check Dr’s office to verify date.
3. If a new physical is required, you must complete and return the MSHSL Sports

Clearance Format time of sign up. Dr., parent and athlete must sign all forms

where signature is required.Dr. office will keep all 3 originals in their file.
A copy of theClearance Form (top page) will be returned to Tartan Activities Office.
4. Any previous MSHSL Dr. form on file at Tartan will be valid until the physical expires.
5. You MUST have a required MSHSL form on file at Tartan to participate in a sport.
Suggestion: Make your Dr. appointment early, if needed, to avoid the fall rush.
*These forms are available all summer outside the Activities Office or online at isd622.org/tartan
under the Activities or Athletics Tab. Click on Activities Office. They will not be mailed to you.

3. The fee is due at the time of sign up. A blue card will be issued after forms and fee are turned in.

The athlete will turn the blue card in to the coach to begin practice(not allowed to practice without blue card).
If any of the above is missing or not completely filled out, your paperwork will not be accepted

and the blue card will not be issued. No forms, no fee = no blue card = no participation

4. If you are on the free or reduced lunch fee, a reduced fee will apply. Bring a copy of your letter from Nutrition
Services indicating your status to receive the reduced fee.

5. Fine Arts students will also need to complete theMSHSL Eligibility Brochure to begin participation.
A blue card will be issued after form and fee (when applicable) are turned into the Activities Office.

6. When an athlete signs up for a second or third activity, no additional paperwork is necessary.

The fee for theactivity is all that needs to be turned in.
