If accepted, I consent to have ______participate in the CCNY weather camp for the two-week period ______to ______listed on the website. I understand that the first week will consist of a day camp during normal working hours on the campus of the City College of New York, and the second week will be split between CCNY and Long Island in the vicinity of Brookhaven National Labs. During the first half of the second week campers will stay in tents at a state park on Long Island. I understand that the camp will be held at no cost to our family except transportation, and that upon acceptance further information and release forms will be requested.
