role specification

implement processes for working collaboratively in order to support young people with learning difficulties and disabilities in their transition to and from Springboard

establish a supportive and professional relationship with students at referral stage and during transition

plan an effective transition between school/alternative provision and college to help to alleviate any anxiety on behalf of the young person and parents/carers

develop and apply robust systems underpinning an effective transition programme

provide weekly key working sessions to young people

maintain detailed and accurate records of student referrals

deal with student enquiries effectively and refer where appropriate.

maintain professional links between schools/alternative provision and Springboard

plan an effective transition between Springboard and future placement such as college, training provider or supported employment

provide monitoring information setting out the transition arrangements for each Springboard student

undertake detailed assessments of young people in their previous educational setting in order to assess suitability for the programme

contribute to the completion of relevant documentation as part of the process for accessing High Needs Funding and meet all Local Authority deadlines

Contribute to the Education, Health and Care Plan (EHCP) process and attend review meetings as appropriate

liaise with external agencies such as Youth Connexions, young people, parents/carers in order that appropriate decisions are taken, learning packages negotiated and progress is reviewed

forge effective professional relationship with external agencies and proactively initiate contacts with new referral agencies

liaise with Springboard staff teams to enable the individual needs of the learners to be met through the sharing of all appropriate information

establish and maintain effective working relationships within the team and communicate any issues regarding referrals or transition arrangements to the Springboard manager or tutor as appropriate

promote Springboard to young people, parents/carers, within the college and to external agencies

Work with colleagues across the Springboard centres to ensure we are providing necessary safeguarding support for students

Operate within the college policies including Equal Opportunities, Health and Safety and Quality Assurance and SEND legislation, Children and Families Act 2014

to assist the manager as required

Person specification

minimum of 2 years’ experience of working and assessing young people with learning difficulties and disabilities

excellent organisation skills, communication and presentation skills, written and verbal

experience of liaising with a range of partners including local authorities, parents/carers, school and colleagues from across the college

knowledge of the SEND Reforms, high needs funding and requirements of Education, Health and Care Plans

Knowledge and experience of Microsoft office applications

Level 4 qualification or minimum 2 years’ experience in a related field

Level 2 qualification in numeracy and literacy or equivalent
