I. POSITION SUMMARY:

The Registered Nurse (RN) assumes professional responsibility for a wide range of client services within the Southern Seven Health Department. The RN is assigned to perform health promotion, health education, and disease prevention, referrals, home visits, developmental screenings, and care plans.

II. ESSENTIAL FUNCTIONS

1.Assists in the determination of eligibility for a wide range of public health and overall agency services.

2.Performs professional nursing functions, including the use of interviewing and public health nursing assessment skills, identifying health, social and other related needs of the client and family; determines urgency of need for service and initiates appropriate action.

3.Assures documentation of service delivery in accordance with professional standards and agency policies to ensure completeness and accuracy of client records and associated databases; uses only agency approved abbreviations. Maintain confidentiality of all client records.

4.Perform home visits according to agency and program policies and procedures.

  1. Coordinates client activities and referrals within the agency across programs as well as with schools and other community agencies as appropriate; maintains knowledge of local and sate program policies and procedures.
  1. Participates in appropriate staff and agency meetings to plan for services and to assure the coordination with community agencies.

  1. Participates in communicable and chronic disease investigation and surveillance services.
  1. Performs delegated medical functions as outline in agency policies such as: taking vital signs, heights/weight, reading TB skin tests, urine analysis-dip, pregnancy, birth control pill refills, Hemoglobin-Cholestec, Emergency Contraception.
  1. Prepares and administers vaccines for an immunization in accordance with medical policies, procedure and standing orders.
  1. Performs lab procedures including venipuncture and prepares specimens as necessary; conducts follow-up on abnormal lab results.
  1. Complies with all standard operating procedures of the health department regarding employee health and safety,, including but not limited to OSHA regulations, CLIA, EPA regulations covering waste disposal, use of appropriate personal protective equipment and completion of incident reports.
  1. Assures that medical supplies and equipment are maintained in an appropriate manner and reports any problems, malfunctions, or supply issues to clinic director in a timely manner.
  1. May be temporarily assigned to work at other clinic sites when workload warrants reassignment.
  1. Assists other nursing or clerical staff as clinic needs warrant.
  1. Provides direction to paraprofessional staff in delivery of client services.
  1. Be familiar and comply with the requirements of the policies and procedures adopted by Southern Seven Health Department as required by the Health Insurance Portability and Accountability Act (HIPAA) of 1996.
  1. All staff are required to attend staff meetings.

III. NON-ESSENTIAL FUNCTIONS:

  1. All duties necessary to meet requirements of Public Health and agency programs.
  2. Participates in Emergency Preparedness Planning/Training.

TRAINING COMPONENT

Participates in continuing education and staff development through seminars, conferences, or workshops at personal or departmental expense (including Indess, LMS, Cornerstone, WIC and Pediatric LMS training).

IV. QUALIFICATIONS:

1. Education, Training, and Experience

A.Certification of graduation from a school or nursing approved by the Illinois Department of professional Regulation, AND

B.Requires an Illinois License as Registered Professional Nurse by examination; or an Illinois license by endorsement within six months of the initial date of employment.

2. Knowledge, Skills, and abilities

A. Knowledge and use of modern office practices and procedures including basic computer skills.

B. Knowledge and use of English language, grammar, and spelling including neat, legible, and appropriate sentence structure. Compose and format correspondence

C. Knowledge and use of appropriate telephone etiquette including use of the agency and personal name.

D. Ability to carry out detailed but basic oral and written instructions.

E. Communicate effectively in written and verbal form.

F. Perform basic arithmetic calculations.

G.Self-motivated, acts independently and initiates programmatic

ideas.

3.Certifications and Other Requirements

A. Required: Criminal background check; employee physical assessment; Valid Driver’s License & Auto Liability Insurance, CPR

B. Recommended: First Aid, Hepatitis B Vaccine

4.Physical Requirements

A. Strength: Lift/carry up to 40 pounds on occasion and 10 pounds

frequently.

B.Movement: Routine:Sit, walk or stand for prolonged periods. Lift horizontally and vertically, bend and stoop, push/pull, twist, carry, crouch, squat, kneel, climb, reach above shoulder level. Hear and understand speech at normal room levels and on the telephone. Drive an automobile.

Occasional:Use near visual acuity to review both written documents and text on computer monitor. Operate a telephone, keyboard and mouse.

C.Blood Borne Pathogen: Routine Occupational Exposure.
