Social Worker Needed for a case load in the Defiance/Williams County area. Will work out of the Archbold, Ohio office: Must hold active LSW or PC.
A consultant opportunity is responsible for providing consultation and supervision to assigned treatment foster parents for Adriel School in accordance with the policies and practices of the organization, the ethical and social consciences of business and society and all applicable laws, regulations and administrative rulings associated with a non-profit organization. A consultant is also responsible for the upkeep of the content of the foster parent and youth files.
1. Provide case management services to youth placed in Adriel homes. See that individual therapy is provided as needed; Establish treatment plans for youth; Coordinate the youth’s treatment program; See that assessments and evaluations are completed as required; Work with the youth’s biological family in those cases where appropriate; Be the liaison and coordinator with the referring agency for the youth; Be available for crises which youth in treatment foster care may experience.
2. Follow the service-delivery rules and policies. Conduct all required home visits, face-to-face contacts, and observations. Provide written documentation of treatment plans, quarterlies, contacts with foster caregivers and youth, and feedback. This documentation will be shared with other treatment team members.
3. Maintain strong, professional working relationships with all internal and external consumers and be an integral member of the entire foster care team. Required expectations include attendance at staff meetings, training, sharing on-call rotation, participation in special events, etc.
4. Assist in the recruitment and development of new foster homes. Oversee that they receive the proper orientation and training.
5. Develop and maintain a working knowledge of licensing regulations, standards, and requirements. As well, demonstrate understanding and adherence to the Teaching-Family Model.
6. Be an integral member of the entire foster care team. This includes attendance at staff meetings, training, sharing on-call rotation, and participation in special events, office coverage, referrals, etc.
Education and/or Experience: This role requires a professional license to practice social work in the state of Ohio. Completion of the State of Ohio Assessor training, Master’s Degree and/or case management experience a plus
