Friends and family often buy personalized dedication ads for seniors to congratulate them for their successes. It’s a great way to commemorate a senior’s high school graduation! Senior ads can be submitted online: go to Monarch HS home page, Activities, then Yearbook. Follow the prompts.

Photos- DIGITAL SUBMISSION

Senior ads can be submitted online: Monarch HS home page, Activities, then Yearbook. Follow the prompts. For each 1/8thof a page, you may submit up to two photographs. Follow these guidelines for photo submissions:

• Submit electronic files of your pictures on a CD

• Files must be jpg (JPEG) format

• Files must be 300 dpi or larger

• IMPORTANT! File names must be in the following format: Lastname_firstname1.jpg, Lastname_firstname2.jpg, etc.

If you are not submitting ads online, you can complete and submit this form with photos on a compact disk.

PLEASE DO NOT SUBMIT HARD COPIES OF PHOTOS; THESE CAN NOT BE RETURNED. But if you would like to come to L205B, you may use our scanner.

Text- TYPE IT OUT

You may submit 175 letters and spaces of text for each 1/8th of a page. Please, do not exceed 175 letters and spaces total for each 1/8thpage. Attach a type-written copy of the text to this form. Please note: the Yearbook staff reserves the right to edit ads for grammatical conventions and length, if necessary.

Please do not submit formatted ads! These will be returned.

Payment- WRITE A CHECK

If you’re not submitting the ad on line, please submit payment in a check, made out to Monarch High School. Please do not submit cash! Payment must be included with order.

Place this completed form, your check, and your CD (with your senior’s first and last name writtenon it with a permanent marker)in an envelope and submit to Ms. Katzive’sroom, L205A/B, by Friday, Oct. 2, 2015.

Ads submitted after this date will be charged a $10.00 late fee, up to one week past the deadline. Ads may also be submitted to the Treasurer’s Office (outside the main office).
