Section 2: Completing the main template
The action plan template comprises 9 columns. Each section must be completed. The following table sets out how each column should be used.
Header / Content / Further guidanceNumber / 1, 2, 3… / This is simply to aid further referencing.
There is no limit to the number of actions that can be included, but it is important to be realistic about how many actions can be completed effectively. In most cases 5 actions at module or course level should be sufficient.

Aim / The ‘aim’ is the intended outcome of the action. The aim will be used to measure the success or otherwise of the action (see success criteria). / Example: The aim may be to reduce number of referrals. This is distinct from the action which might be determined to deliver the aim.
Action(s) / The specific actions through which the aim will be addressed. Each action should be numbered separately to aid clarity.
In the example on the right the single aim of ‘reduce number of referrals’ has generated three specific actions which are expected to deliver the aim.
Where the action is to keep an issue under review or to reflect further, it should be clear how and where this will be undertaken and reported (e.g. module team to reflect on continued use of lectures to deliver content and to report outcome to Course Board in June). / Example:
1) review and revise assessment task (may require committee approval).
2) introduce additional revision session in week 10.
3) Create on-line formative test for week 7.
Timescale / The target date by which the action(s) should be completed.
Action By / This should state where the responsibility for taking the action lies, which might be an individual role or the identification of a group of staff which carries collective responsibility for the area. This should identify the role of the individual or the group and may include individual names. The aim is to be unambiguous about who has to undertake the work.
Responsibility / This is the person who has overall responsibility for ensuring the action is undertaken. In many cases this will be the same person as ‘Action by’, but at course or programme level it may be the Course or Programme Leaderwhere actions are being taken by other colleagues.
Success Criteria / Success criteria should be precise and (ideally) measurable outcomes against which success can be judged. The criteria relate to the ‘aim’ and provide information on how we will know it has been achieved. There may be multiple criteria to support a single aim. / Example:
- 50% reduction in referral rate
Monitoring / The monitoring section should include:
- What is being monitored
- Where it is being monitored
- When it is being monitored

Where: Course Board
When: Prior to start of term
Evaluation / The evaluation is linked to the aim and listed success criteria. This section should indicate:
- When evaluation will take place
- Where the evaluation will be considered
- What evidence will be used for the evaluation.

- Student performance data
