Enterprise Chart

Overview

The Enterprise Chart enables a multi-chart view of patient clinical data from multiple practices at the same time. Although this feature enables the clinical user to view data across the practice boundary within the enterprise, Enterprise Chart still provides practices the ability to segregate financial data at the practice level.

This section will demonstrate functionality that is specific to the Enterprise Chart ONLY. If your practice is not using the Enterprise Chart option, you may skip this section of the EHR Training Manual.

Key Features:

• Ability to turn on Enterprise Chart at the Enterprise Level.

• Ability to default new patients as “opt - in” or “opt - out” of participation in the Enterprise Chart functionality. If a patient opts out of the program the provider will only see information entered in the current practice.

• Ability for patient to “opt - in” or “opt- out” on the fly. This enables the patient to change their mind.

• Graphical indicator, displayed in the patient lookup screen that identifies if the patient participates in Enterprise Chart.

• There is a “my practice” feature that enables users to see all encounters across the enterprise of just the ones from their practice.

Enterprise Chart Patients

Enterprise Chart Patients are those patients whose clinical data is viewable across the entire Enterprise, regardless of specific Practice.

Once you enable the functionality, you have the following options for selecting the availability of all existing and all new patients via the Enterprise Chart:

·  Enroll existing and/or new patients on an individual case-by-case basis

Visual Representation of Enterprise Patients

All patients enrolled with the Enterprise Chart are represented with a different icon in the Patient Lookup window (see image below):

In the image above, Andrew Doe is a regular NextGen patient, while John Doe and Jane Doe are both enrolled into the Enterprise Chart functionality, enabling you to view their clinical data across all of the Practices within your Enterprise.

Workflow Tasking

·  In order to send and receive tasks between practices, a user must be a user in those practices. For example, in order for a family practice to send a task to a podiatrist the podiatrist would need to be added to the family practice.
(NOTE: The Family practice manager must fill out a Track_IT user access request to add the specialist to that practice.)

Enrolling (New or Existing) Patients into Enterprise Chart

Who will be responsible for setting up the Enterprise status for a patient? That is, will the “original” Practice or the “new” Practice be the one to set the Enterprise status? In the end, it will be up to the Practices to decide on this.

Whatever decision is made regarding who will set the Enterprise status for a patient, the following steps to enroll new or existing patients into Enterprise Chart will be the same although the workflow may be different between practices.

Enrolling a New Patient into Enterprise Chart

To Enroll a New Patient into Enterprise Chart:

1.  Click the Patient Selection icon on the Top Tool Bar to access the Patient Lookup window.

2.  Click the New button.

You are prompted with the Add Patient Information window.

3.  Enter all required information.

4.  Select the Enterprise Chart checkbox.

5.  Click the OK button.

You have enrolled a new patient with Enterprise Chart.

Enrolling an Existing Patient into Enterprise Chart

To Enroll an Existing Patient into Enterprise Chart:

1.  Click the Patient Selection icon on the Top Tool Bar to access the Patient Lookup window.

2.  Perform a search for the desired patient.

3.  Select the patient and right-click on the patient name.

4.  Select the Open command from the Menu list.

You are prompted with the Update Patient Information window (see image below).

5.  Select the Enterprise Chart checkbox.

6.  Click the OK button.

You have enrolled an existing patient with Enterprise Chart.

Enterprise Chart Preferences

The Enterprise Chart functionality provides you with an additional NextGen EHR User Preference to default the display of Encounters in the History Tool Bar to the Current Practice. Once you enable this preference, only Encounters from the Practice that you are currently logged into are displayed in the History Tool Bar.

Setting up Enterprise Chart Preference

1.  From the Main Menu, select Tools > Preferences. You are prompted with the Preferences dialog.

2.  Select the Main Toolbar tab.

3.  Select the Show encounters for current practice only checkbox.

4.  Click the Apply button.

5.  Click the OK button to accept the changes that you have made and exit from the Preferences dialog. To exit without saving the changes, click the Cancel button.

History Tool Bar and Enterprise Chart

With the Enterprise Chart option turned on, the History Tool Bar displays all Encounters for the selected patient.

Note: Only patients enrolled into the Enterprise Chart display Encounters from all Practices.

All Encounters from a Practice different than the Current Practice are displayed with a blue folder icon (see image on previous page).

Important: All Encounters from outside of the Current Practice are automatically locked. You cannot unlock such Encounters. If you need to modify or add new patient data, log into the pertinent Practice and access the desired Encounter.

Display of Current Practice’s Encounters Only

You have an option to display ONLY the Encounters from the Current Practice

To Display Current Practice Encounters Only:

1.  Click the Toggle History Toolbar icon on the Top Tool Bar to access the History Tool Bar (see image below):

2.  Select the My Practice checkbox.

When the My Practice checkbox is marked, only the encounters for the current practice is displayed in the History Tool Bar. However, since Enterprise Chart is still “on”, the information from the different practices can still be viewed in the templates although the encounters from those different practices are no longer viewable in the History Tool Bar.

Note: If the Enterprise Chart Preferences is turned on, the My Practice checkbox is selected by default.

Only the Encounters from the Current Practice are displayed in the History Tool Bar.

Enterprise Chart Patient Data

The Enterprise Chart enables you to see all clinical data for the selected Enterprise Chart Patient, regardless of the Practice. However, all patient clinical data from other Practices is available in view-only mode - you cannot add or modify the displayed information. The following chart demonstrates the available data and what you can do with each item.

Item: / Availability of Data from other Practices:
Documents / You are able to view, print and/or fax all of the Documents.
Images / You are able to view, print and/or fax all of the Images.
ICS Images / You are able to view, print and/or fax all of the ICS Images.
Procedures / You are able to view all of the patient's Procedures.
Problems / You are able to view all of the patient's Problems. You will not be able to resolve a problem if problem is from a different practice.
Allergies / You are able to view all of the patient's Allergies. You cannot resolve an allergy from a different practice using the Allergy module. However, you can resolve an allergy from a different practice using the Allergy template.
Medications / You are able to view all of the patient's Medications. NextGen EHR performs all pertinent Drug/Drug, Drug/Allergy Interactions, as well as Medication Duplication and Medication Contraindication checks. You can view all meds across practices. You can inactive meds across practices as long as “prescribed elsewhere” has not been marked. You will not be able to renew, add a stop date, print, or fax meds across practices.
Item: / Availability of Data from other Practices:
Labs / You are able to view all lab orders and results across practices.
Inbox/Work Flow Module / You are able to send tasks with or without attachments across practices. Remember, however, that one must be a user in those practices to be able to send and view tasks across those practices. For example, in order for a family practice to send a task to a podiatrist the podiatrist would need to be added to the family practice. Only attachments such as documents and scanned images that are sent across practices can be opened for review. Attached charts sent across practices cannot be opened for review.
Alerts / You will be able to view EHR alerts documented from a different practice.
Chronic Conditions / You are able to view all chronic conditions across practices.
Family History / You are able to view all family history across practices.
Social History / You are able to view all social history across practices.
Past Medical History / You are able to view all past medical history across practices.

Enterprise Chart Patient Data

Medication Module:

After a patient is on Enterprise Chart and when you open the Medication Module, the highlighted medication (see image below) will show in the medication summary grid the original provider information as a unique ID#. Also, in the medication details area, the current provider’s name will show in the Provider field and the original ordering location will show in the.Location field. The current provider’s name is not saved permanently unless that current provider makes any modification to the existing medication order. When a modification is made to an existing order, then that current provider’s name and current location will be saved with that order.
