Student and Parent: Pleaseunderstand that a full commitment must be made to all rehearsals and performances, and that the music must be prepared thoroughly, in advance of the first rehearsal. Failure to attend any rehearsal or performance will result in dismissal. Furthermore, by participating in a SCMEA sanctioned event, the performer’s image, or likeness may be used in SCMEA’s publicity efforts.

Teachers: Please do not sign this form unless you have fully reviewed the SCMEA Attendance Policy with your students and have confirmed that thereare absolutely NO school related performances or rehearsals or other conflicts to prevent full commitment. Also, by signing you acknowledge the SCMEA Chaperone Policy for this festival and will ensure that all participating students are chaperoned by an official school chaperone as per SCMEA Policy. Please do not sign this commitment form if there are any questions with regard to attendance or chaperone policies.

This form must be signed and returned to the child’s music teacher by:December 3rd, 2017,or he/she will forfeit the nomination into the SCMEA Ensemble.
