FORM F4: WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to participate in any way in the SINGAPORE DRAGON BOAT FESTIVAL (SDBF) 2013to be held at Bedok Reservoir Singapore on 6th and 7thJuly 2013, I the undersigned, intending to be legally bound hereby for myself, my heirs, executors and administrators, do waive and release my and all rights and claims for damages I may have against SINGAPORE DRAGON BOAT ASSOCIATION & the CO-ORGANIZERS, their directors, partners, members and sponsors.
- Agree that prior to participating, I will inspect the facilities and equipment to be used and if I believe anything to be unsafe, I will advise my team manager, coach or sponsor of such conditions and will refuse to participate.
- Acknowledge and fully understand that myself and each participant will be engaging in activities which involve risk of serious injury, including permanent disability and death and severe social and economic losses which might result not only from my own, but also my actions, inactions or negligence of others, the rules of play, or the condition of the premises or any equipment used. Further, that there may be other risks not known to me or not reasonably foreseeable at this time.
- Assume all the foregoing risks and accept personal responsibility for the damages following such as injury, permanent disability or death.
- Release, waive, discharge and covenant not to sue SINGAPORE DRAGON BOAT ASSOCIATION and the Organizers, Co-Organizers, administrators, directors, agents, independent contractors and other volunteers or employees, if any, of the organization, other participants, sponsoring agencies, sponsors, advertisers, and owners and leasers of the premises used to conduct the event, including Public Utilities Board, the National Parks Board, the Singapore Land Authority or any other Agencies (hereafter referred to as ‘releases’) from demands, losses or damages on account of injury, including death or damage to property, caused or alleged to have been caused in whole or part by the negligence of the releases or otherwise.
- For Parents/Legal Guardians of Participants under 21 at time of registration. This is to certify that I, as parent/legal guardian with legal responsibility for this participant, do consent to his/her release as provided above of all the releases and for myself, my child and my heirs, assigns and next of kin, I release and agree to indemnify and hold harmless the releases from any and all liabilities incident to my minor child’s involvement or participation in the event, even if arising from the negligence of the releases, to the fullest extent permitted by law. I have read, understood and intend to be bound by this Participant Agreement.
