04 INSTRUCTIONS FOR REQUIRED ATTACHMENTS

I. Agency Organization Chart

Provide an organization chart of the overall agency. If the project is a subset of the agency, highlight the position of project staff within the overall agency.If the agency is partnering with other agencies for this project, include an organization chart for the project team, illustrating the relationship between the organizations.

II. Board of Directors Roster

Provide a roster of the agency's Board of Directors, including the following information.

4. Phone number (indicate whether it is business or home)

5.FAX number (indicate whether it is business or home)

6.E-mail

B.Number of Board members required by your by-laws.

C.Demographic profile of overall board, including such characteristics as gender, age (e.g., under 30, 31 - 40, 41 - 50, 51 - 60, 61 - 70, 71+), ethnic identity, sexual orientation, physically challenged etc. where data is voluntarily available.

III. Letters of Agreement

If the applicant will be participating in a collaborative and/or subcontracting relationship, provide a Memorandum of Understanding, work agreement, or letter from the partnering organization which documents the relationship and defines tasks for which each party will be responsible.

IV. Letters of Support

Provide three (3) letters of support. If applicants are funded by City of San Francisco, one of the three letters must be from a City Department that provides funding.

V. Financial Report

Provide one copy only of the FY 2009-2010 Annual Financial Audit Report prepared by independent certified public accountants, with balance sheets and related statements of revenues, expenses and changes in fund balances, functional expenses, and cash flows.
