When there are multiple consumers on an incident, what transfers over to each consumer’s incident?
Because of the way the system is set up and the flow of the Incident Report, we have to enter in each consumer that was involved in the incident separately, into each section that requires it. For instance, if an Incident involves 3 consumers you will need to enter the consumers’ names into the Initial Incident section and then the Additional Questions section (if needed).
If a consumer is in more than one program, is there a way to alert the other program of the consumer’s incident?
A consumer is enrolled in more than one program. What is the process of completing an incident report when consumers are enrolled in more than one program?
Because Safety Zone doesn’t have a consumer database, every time a UIR would be filled out, you are entering a consumer into the system for the first-time. We can run reports to see which consumer have multiple incidents attached to their name, so just make sure that the name of the consumer is spelled correctly and that their Carelogic ID is entered into Safety Zone correctly as well.
What will happen if I choose the wrong program?
If the wrong Program is chosen, you can send in a request to , and someone will make sure that the Incident gets routed to the proper Program.
How do we upload documents?
When you finish the incident and hit ‘Save’, you will be asked if there ‘are any attachments for this event?’
Click on ‘Yes’ and enter in the Description, Category and Attachment and then ‘No More Attachments’, if there are no other attachments to add.
If there is another attachment to add, click on ‘Add Attachment’ and proceed to add any other attachments that are necessary.
When there is an injury, where do we include the body scan?
Can we add a prompt to indicate body scan?
How do I make a consumer inactive once they have been discharged?
Is there a begin time and end time for BPC? How do we capture this data? This is required from a licensing perspective.
When do I complete a UIR (what is requirement)?
Can we send DDD forms from Safety Zone?
When do we send Appendices and where do we find them? This question applies only to DMHAS programs.
