Project Graduation Waiver (Due May 1, 2017)

I give permission for my child ______to attend the RHS Project Graduation 2017 and participate in all activities on Friday, May 26, 2017.

I understand that the students will be chaperoned during the event and that normal precautions will be taken to ensure their safety and well-being.

I agree to release Rockdale Independent School District, Rockdale High School, Austin’s Park ‘n Pizza, Clark Travel and volunteers/chaperones from all legal liability during this activity.

In case of emergency, I give my approval and authorization to render first aid treatment. I agree to accept responsibility for payment of all charges incurred during their medical treatment. I further agree to release, hold harmless, and indemnify the Rockdale HS Project Graduation sponsors and Rockdale Independent School District from liability, damages and/or claims made by any person related to the injury, treatment, or care of my child.

This authorization begins on Friday, May 26, 2017 for the duration of the Project Graduation Celebration sponsored event, and is made pursuant to chapter 35 of the Texas Family Code.

*Project Graduation is a substance free celebration honoring the 2017 graduating class of Rockdale High School. Understand that any student who does not conduct himself/herself properly will be sent home at the parent/guardian’s expense. The parent will be called to pick the student up from the event.

*This is a drug and alcohol free event. Students caught with illegal substances will be sent home at the parent’s expense. If a parent refuses to pick up a child, or if we can not contact a parent, the authorities will be notified.

Students must meet at the RHS parking lot by 10:00 pm to attend the event. Transportation will be provided by Clark Travel to and from the event. The bus will not wait for late-arriving students. Once the student enters Austin’s Park ‘N Pizza, they will not be allowed to leave. The students will return by the RHS parking lot Saturday, May 27, 2017 at approximately 8:00am. Students will not be admitted at Austin’s Park ‘N Pizza without picture ID.

Students will not be allowed to attend the Rockdale HS Project Graduation Party unless this form is signed by both the guardian and student and returned to the school.
