DONCASTER FALLS RISK ASSESSMENT SERVICE LAUNCH and TRAINING EVENT – 23rd JANUARY 2014

Invitation for all Pharmacy Staff organised jointly by

Doncaster Local Pharmaceutical Committee and Doncaster CCG

Doncaster LPC has been working with the CCG to develop a falls risk assessment scheme. This scheme is based on some research undertaken by Doncaster LPC member, Claire Thomas. Further details of the service will be provided in the service specification document which will be sent out as soon as it has been signed off.

You may wonder why the delay and indeed the short notice for the training event – this is because this service has been developed and launched in a very short time period, because the funding being used is coming from the Winter Pressures funding released by the Department of Health in December.

In summary the aim of this service is to enable pharmacy to play an even stronger role at the heart of more integrated out-of-hospital services for patient’s that deliver better health outcomes, more personalised care, excellent patient experience and the most efficient use of NHS resources.

This new community pharmacy service aims to address the issue of primary prevention of falls and fractures: identifying patients at risk and preventing the first fall/fracture

This is a pharmacist based service, but does require the support of the pharmacy team. It is intended that the service will be provided as an add on to an MUR in most cases, but could equally sit aside an NMS or other patient consultation or on its own.

Attendance will be on pre-booking and places will be on a first come first served basis. As this is a pharmacist based service priority will be given to pharmacists and guidance given for them to cascade the operational aspects to their teams.
