POST: TransitionSupport Worker

RESPONSIBLE TO: Transition Development Manager (Line Manager) – Residential Managers (Key Stakeholder)

LOCATION

  1. The post requires the post holder to work flexibly between the Trusts residential services and Aftercare Service.

PURPOSE OF JOB

  1. To provide direct support to young people, in managing with confidence all stages of transition across the Trust’s services. From entering residential child care to living independently in the community.
  2. Support the delivery of residential care plans, in partnership with Residential Care Workers.
  3. Provide direct visiting support to a proportionate number of young people living semi independently in the community.
  4. To support the Transitional Manager in developing and maintaining a range of services and supports in assisting young people to make smooth transitions through the Trust’s residential accommodation into settled accommodation in the community.
  5. To work in partnership with the Trust’s residential and aftercare services to establish good working practices in supporting the preparation of young people for a life after care.
  6. To co-ordinate supports for young people in accessing education, training and employment to ensure smooth transitions.
  7. To ensure practices within the services remain young person centred and that young people feel fully supported in the transitional period from residential care to independent living.
  8. To develop positive working relationships with young people to encourage feedback and evaluation of transitional experiences.
  9. To ensure clear recording and monitoring of the work output and impact of the post in line with funding expectations.

RESPONSIBILITIES AND TASKS

  1. The post holder will be responsible to the Transitional Manager as his/ her first point of contact and line management responsibility, however the nature of the post dictates that the post holder will work across the Trusts services.
  2. The Post holder will work in close Partnership with the Residential care workers and Aftercare Support workers, in developing transitional preparation and pathways for young people within our residential services.
  1. The post holder will be responsible for assisting the Transitional Manager in developing a transitional flat within one of the Trusts residential units, with the intention of using this for the purpose of close support transitions.
  2. Providing individualised one to one support to residents within the transitional flat, to develop resilience and confidence as well as practical independent living skills.
  3. To support the Transitional Manager in developing a robust training programme incorporating aspects of personal development and tenancy preparation in partnership with the Aftercare Manager and Residential service Managers.
  4. Deliver the above training programme to young people in partnership with the Residential, Aftercare and Group work staff within the Trust.
  5. Work with the support and guidance of the Transitional Manager.

OTHER DUTIES AND RESPONSIBILITIES

  1. To attend regular team meetings and contribute to practice discussions and team development.
  2. To ensure that all work undertaken within the course of the role, adheres to the Health and Safety policies and procedures of the Trust to protect the welfare of self and others.
  3. Work in accordance with the Policies and Procedures of the Trust.
  4. To undertake professional supervision at pre-arranged intervals as directed by the line manager.
  5. To undertake such other appropriate duties as the Board of Trustees or Chief Executive shall determine.
  6. To ensure all aspects of practice adhere to the SSSC codes of practice for social service workers, and the relevant expectations of the Care inspectorate.
  7. To report, without delay all concerns relating to Child Protection and Adult Protection.
