Resident Lifting/Transfer Plan

Purpose

To establish procedures to insure the safety of our employees at ______who assist our residents who require lifting or transfer assistance.

Administrative Duties

Hazard identification, analysis and recommended action will be done by the employee based Safety Team working with the Administrator and Director of Nursing (DON)

The Administrator shall be responsible and have authority for compliance to this program.

The Safety Team, Administrator, DON and Charge Nurses will be responsible for monitoring compliance. The Administrator is responsible for follow up on deficiencies.

Plan Components

·  Establishing each residents current lifting/transfer requirements

·  Training staff to safely perform the lifting/transfer requirements

·  Communicating the lifting/transfer requirements to staff

·  Communicating changes of condition

·  Enforcement of the established lifting/transfer requirements

Establishing Lifting/Transfer Requirements

Each resident’s mobility upon admission will be determined, documented, and communicated to all nursing staff personnel.

Resident lifting/transfer assistance needs will be determined by the following method. All new residents will be initially evaluated by ______based on medical history, resident and resident family interviews, and observation. In cases where ______is not available designated backups for each shift will be ______

Following each shift, the nursing staff will be asked to provide updates and/or report any changes in condition to the incoming charge nurse. The reporting will be documented by (how) ______. Nursing Staff Training

Employees will be trained for all levels of resident lifting/transfers including use of all lifting assistance equipment. The training will be documented in writing and placed in the employees personnel file. All employees will receive retraining on an annual basis at a minimum.

In addition to training on proper lifting/transfer procedures, employees will receive training on how to properly identify and communicate changes in condition that my change the residents lifting/transfer assistance needs.

All employees will sign a statement acknowledging that failure to follow this policy is cause for disciplinary action including employment termination.

Plan Review

This plan will be reviewed annually by the Safety Team, Administrator and DON to determine if any updates or changes are needed.
