1. I have reviewed paragraph 11.5.3 of reference (a) and I hereby certify that Last Nameis required to perform frequent and regular in-flight duties (averaging at least four flight hours per month) that are essential to the accomplishment of this command’s mission. in-flight duties will commence on Dd, and will continue throughDd, or detachment from this command, whichever is sooner.

2. In the event that Last Name’s assigned duties no longer require frequent and regularparticipation in aerial flights sooner than the end date listed above, I will notify PERS-435 to terminate DIFTECH orders in accordance with reference (a).

3. I request that DIFTECH orders be issued for Last Name, and that be considered eligible for Hazardous Duty Incentive Pay (HDIP) for flying duty in accordance with reference (b), effective between those dates specified in paragraph 1 above.

4. is aware that it is responsibility to submit verification of flight hours annually and/or at other times as directed by reference (c), by annual NAVADMIN message, or by the Program Manager (PERS-435); and that any HDIP disbursements received, if not validated with properly submitted flight hour verification, will be subject to recoupment.
