Payment PolicyI understand thatPark’s Royal Body Works, Inc. (PRBW) accepts Original Insurance Checks, Cashier’s Checks, Cash, Debit Cards, Visa, MasterCard, and Discover. Creditcard payments will be charged a 3% processing fee. Personal checks are accepted with prior approval. Payments from 3rd partiesare not accepted. If a 3rd party is paying for my repair, it is my responsibility to collect payment from them. My vehicle will not be released until payment is made in full, or PRBW has received verification from an insurance company that payment is being sent directly to them.

Estimatesare prepared based on visual damages only. If hidden or additional damage is found, asupplemental billwill be created and whoever is responsible for payment will be contacted. If a delay occurs I will be notified. All parts prices are subject to invoice.

I understand that PRBW’s final invoice for repairs of my vehicle may not match that of the insurance company’s estimate as to parts, labor or procedures used to repair my vehicle. I understand that parts prices are based upon recent Estimating Software; however prices may vary due to supplier costs. I understandthat repair techniques employed are based upon PRBW’s judgment as to the best means of effecting a high quality repair, and that PRBW labor charges are based upon hourly time allowances as given in the collision repair industry estimating software, and their experience, and may be more or less than the amount of actual time. I understand that photos of my vehicle, before and after repair, may be used in training and marketing materials.

I understandthat all estimates and final invoices are based upon flat rate hours as is customary in the collision repair industry, not actual time spent on repairs. I understand that a flat rate hour is a unit of time given for a certain repair operation and that actual time spent on a repair may vary greatly from the flat rate time given to a labor procedure depending on technician experience and equipment provided and/or training.

An Express Mechanics Lienis hereby acknowledged on the below mentioned vehicle to secure the amount of repairs. I further agree to pay all of PRBW’s reasonable attorney’s fees and court costs in the event legal action is necessary by PRBW to enforce this contract.

I have readthe above paragraph(s) and hereby authorize Park’s Royal Body Works, Inc. (PRBW) to repair my vehicle as described in the “estimate” and order any necessary parts prior to my scheduled appointment date. I agree that, if I fail to have my vehicle repaired as scheduled, I will be responsible for a 10% re-stocking fee and any freight charges on the parts and a $50.00 administrative fee. I agree that PRBW is not responsible for loss and/or damage to my vehicle caused by fire, theft, or any other cause beyond its control and for loss of articles left in the vehicle, or for any delays in repair completion. I further understand that PRBWwill not be responsible for any rental car charges I incur. I also grant permission to PRBWemployees to operate my vehicle for the purpose of test driving, etc. during the repair process.

By signing this documentI authorize the paying party to pay Park’s Royal Body Works, Inc. directly and hereby nominate, constitute, and appoint Matt Thornton or Cynthia Thornton of Park’s Royal Body Works, Inc. my true and lawful Attorney-In-Fact for me in my name, place, and stead, and on my behalf, and for my use and benefit to transact all of my banking business concerning endorsement of original and/or supplemental drafts (checks) for original and/or additional repairs to my vehicle made during the course of this repair.
