Required Admission Documents for a Foreign Healthcare Service Contractor

DOCUMENTS REQUIRED FOR ADMISSION

FOREIGN HEALTH CARE SERVICE CONTRACTOR

CHAPTER 48.44.RCW, CHAPTER 284-43 WAC

1. UCAA Expansion Application: http://www.naic.org/industry_ucaa.htm

2. Include the following in addition to the UCAA application per RCW 48.44.040

· Area of operation;

· Documentation that the health care service contractor has an initial net worth of at least three million dollars and shall maintain the minimum net worth required under RCW 48.44.037;

· Type or types of health care services provided;

· A copy of all contracts being offered and a schedule of all rates charged;

· Plan for handling insolvency which meets the requirements of RCW 48.44.055;

· Per RCW 48.44.030 a funded reserve shall be created. The statutory reserve may be a surety bond or $150,000 deposit held in trust in a trust account in a bank meeting the requirements of RCW 48.16.070;

· A list of participating providers.
