Registration Information

Please forward this registration form to any other guests (parents, family, mentors, etc.) who plan on attending the symposium, who are not part of your school’s registration and payment. They will be required to fill out the form and send it back to us by the required deadlines listed below. If preferred, you may email ( ) your registration first, and then send your payment by mail. However, the payment deadlines must be strictly adhered to.

**All attendees (teachers, students, guests, etc.) MUST also register through the AEOP CVENT site. This site is being used to collect important demographic information that is REQUIRED for participation in the program. If you do not complete this part of the registration, we will be informed, and your registration will be viewed as incomplete until you fulfill this requirement.

Registrations and full payment for the March 7th and March 8th, 2017 Upstate New York Junior Science and Humanities Symposium are due Friday, February 17, 2017. There will be a late registration deadline of Monday, February 27th. All late registrations will be charged a higher fee (see below for details). Registrations sent after this date will not be accepted.

Substitutions of participants’ names can be made up until Monday, February 27, 2017. Refunds in full will be made for cancellations until Monday, February 27, 2017. After this date no refunds will be granted or accepted as we are required to give the caterers final numbers for meals.

Registration/ Payment Dates and Information

Ø  Regular Registration – Friday, February 17th, 2017.

Ø  Late Registration – After February 17th – Monday, February 27th

Ø  Payments – All registrations including payment must be postmarked (envelopes, not payment) by Friday, February 17th for the early registration price and Monday, February 27th, 2017 for late registration pricing. Acceptable payment types include purchase orders or checks.

Registration Type: Please write the following abbreviations next each participants’ name to identify registration types: (F) Full; (O) Day One Only; and (V) for Visitor. Also, indicate who your student presenters are by writing (S) Speaker or (P) Poster next to their names. Student presenters must register as Full Symposium attendees, and are required to stay through the awards ceremony on Tuesday night. Absences must be authorized by JSHS Co- Director, Dr. Donald Orokos.

Sub-Regional and Upstate New York JSHS Participant Rules of Conduct

Signatures from a teacher, a student, and the principal are required at the bottom of this form

Teachers, it is your duty to register yourself as well as other teachers, etc. from your school and all of your participating students. Students will either be observers or presenters.

Student attendees listed on the registration form must have parental and school permission to attend the March 7th and March 8th, 2017 Upstate NY JSHS to be held in Albany, New York. Student attendees must complete the Upstate NY JSHS, University at Albany, State University of New York Consent and Release form. ** Please collect and submit the forms promptly before the symposium. Presenters have been asked to send their forms along with their papers, abstracts, & certifications, 72 hours after the sub-regional symposium.

Conduct

Each attendee (teacher, student, parent, guest, mentor) is expected to abide by the following Rules of Conduct established by administrators of the Upstate New York Junior Science and Humanities Symposium. Please read thoroughly and sign below.

The symposium is a formal, educational event. Attendees are expected to be respectful of their peers, speakers and other attendees. Should disruptive behavior arise, the staff and designated chaperones of JSHS will resolve the problem in a fair and effective manner.

o  The use or possession of alcohol, drugs or hallucinogens, in any form, will result in immediate dismissal of a student and his/her return home.

o  Should it become necessary to dismiss a participant for violating the Rules of Conduct, the parent or guardian will be liable for costs of transporting the student home.

JSHS is a school related, not an individual, event. Therefore, students must be chaperoned for the entire symposium by a teacher from their school.

**Parents and teachers are welcome; however, only a teacher can be the designated chaperone. **

Lodging

All room reservations and charges for JSHS are the responsibility of the teacher. Room reservations for all participants must be made directly with the Hotel. Please note that transportation to and from the hotel, if needed, must be arranged and paid for by the chaperone.

All hotels are located directly across from the University either on Western Avenue or Washington Avenue. Room reservations for all participations must be made with the hotel(s) by the date listed next to each. Reservations made after these dates cannot be guaranteed.

Students are housed three or four per room with two double beds. The hotel will not match roommates from different schools.

Bring sleeping bags. Cots are not available.
