Recognition Programs
- The “High 5” programwas developed as a tool to encourage peer-to-peer recognition. In this program, staff members thank their co-workers or other CVHP staff members within the organization for making a difference in everyday work life and in the patients we care for. The gift cards can be taken from the superman box, but a VIA form needs to be used. Please see attached instructions.
- Good Catch –on detecting problems before harm reaches our patients. A program shows that focuses you are a champion for patient safety, your efforts have truly made a difference to those whose lives are in your hands, for taking the extra step and being a positive role model for others. Get the $2.00 coupon. Look for catching medication errors, CPOEs, hand hygiene, etc. Managers write up why the person deserved a “good catch” recognition. Entered into a drawing, monthly, win movie tickets, In’n’Out gift cards, etc.
- Values in Action (for all customers)– recognition when an employee “goes the extra mile to provide excellent service.” Given a VIA form with a $2.00 coupon attached to the certificate.
- The VIA program emphasizes that our customers, whether they are patients, visitors, vendors, or physicians-or even fellow employees-have a choice about coming to CVHP, and we are proud that they selected us as their healthcare provider. It is the expectation of the VIA program that all individuals associated with CVHP live our values and mission. These behaviors and actions will enhance the patient experience, improve their perceptions about our care and services, and to improve interrelationships between all members of the organization.
- Performance Recognition
- Employees are recognized as outstanding employee by going above and beyond their expected level of performance and living the values of Citrus Valley Health Partners in their interactions, deeds and contributions.
- Must have exhibited one or more of the following values:
- Respect
- Excellence
- Compassion
- Integrity
- Stewardship
- Nomination:
- May be made by anyone (visitor, volunteer, patient, physician, employee or other person)
- Is documented on the VIA recognition form.
- Awards:Individual
- VIA recognition form is completed by the person making the nomination.
- Form is sent to the Chief/Vice President/CNE/Director of the person being nominated.
- Chief/Vice President/CNE or Director reviews and signs form, removes top page and forwards it to Human Resources (CVHP). The bottom page is forwarded to the recognized person via his/her Department Director.
- Human Resources (CVHP) forwards copy to be filed in personnel file.
- Employee removes VIA coupon and presents it when making a purchase. The coupon will be honored at face value at Cafeteria and Gift Shop.
- Quarterly
- The recipients will receive:
- A recognition pin.
- Recognition in the organizational newsletters/Monday Memo.
- A $300 value gift from the appreciation catalog.
- Recognition at annual Service Awards event.
- Annually
- One of the quarterly employee winners at each facility will be selected as an Employee-of-the-Year. The person selected for each facility will receive:
- Recognition at the annual Service Awards event.
- An Employee-of-the-Year plaque.
- Recognition in the organizational newsletters.
- Amonetary gift.
- Values in Action AKA Superman Box(leadership given on the spot gift card rewards) – Employee Performance Recognition . The individual VIA form copy is given to PR to replenish the gift cards given.
- Employees are recognized as outstanding employee by going above and beyond their expected level of performance and living the values of Citrus Valley Health Partners in their interactions, deeds and contributions.
- Must have exhibited one or more of the following values:

- The Daisy Award
- Nurse’s clinical skill and especially her/his compassionate care exemplify the kind of nurse that our patients, families, and staff recognize as an outstanding role model.
- Makes patients and families feel safe and comfortable
- Provides individualized, compassionate and patient centered care to the patient and family
- Goes above and beyond your expectations
- Role models positive service behaviors (i.e., Class ACT: Attitude, Courtesy and Teamwork)
- Perfect Attendance – Employee has no unscheduled PTO days during calendar year
- Employee of the Quarter –using the two VIA forms/patient letters/compliments.
- Employee of the Year – comes out of Employee of the Quarter. Recognized at the Annual service award luncheon.
- Years of service recognition – 5 years of service award brunch, then every 5 years afterward to 35 years, and 35 years plus get a special recognition lunch w/Rob Curry and Paul Heredia.
- Elvia Foulke award – Nominations are solely chosen through Elvia Foulke and Administration. Administration does the presentation. Includes a trophy to recipient.
- Gold Star Partners – nominated by Physician Coaches committee for employees who have shown exemplary service while doing their jobs. Certificate of recognition, signed by Dr William Choctaw, is given to the employee through his/her director.
