Reading Growth Charity Pledge Form

Student Name:

Charity Name:

Beginning Reading Level: Ending Reading Level:

Reading Level Growth: (to be multiplied by pledge amount)

The reading level growth is the total number of reading levels the student grew throughout the year. This will be calculated in May. Sponsors pledge a dollar amount that they promise to donate for each reading level that student grows by May. Pledge money will not be collected until we know the total growth in May; however, students can start asking sponsors for pledges now. When money is being collected in May, we will need to be sure that all donations are in the form of a check made out directly to the charity of the child’s choice. As the students start getting sponsors and pledges, use the chart on the back of this paper as documentation.

For further explanation, here is an example:

Johnny Prince has chosen Children’s Hospital as his charity of choice. He began the year at a reading level B. He starts asking his friends and family to sponsor him, by pledging to donate money to his charity for every reading level he grows by May. One sponsor, Johnny’s grandma, pledges $5.00. Come May, Johnny is at a reading level J. That means he grew 8 reading levels! Johnny’s reading level growth of 8 levels times the $5.00 pledge his grandma made equals a total of $40.00. Grandma writes a check to Children’s Hospital for $40.00. Way to go!
