Re:Report of Progress on Balancing Incentives Project Grant

CDPAANYS efforts related to the peer mentoring project continue to move forward. As evidenced in the Interim Report, the grant’s primary objective of demonstrating through post-interaction surveys that peer mentoring is an effective tool in getting people to choose and remain in CDPA, is proving a success.

We are hitting all deadlines and the survey results from calls are extremely positive.As of January 31, 2015, the peer mentors have handled a total of 765 calls, surpassing the lower level target and achieving 77% of the upper level target. It is notable that approximately 30% of calls have been from consumers/DR’s who are using the service more than one time, demonstrating that consumers and DR’s are using the service to help manage their programs more effectively.

As noted, all other grant targets are being met, and importantly, evaluation data continues to show that the project is successful in meeting its goal of proving the effectiveness of CDPA in helping people to choose and remain in CDPA. In the coming months, we will focus on collecting and analyzing the remaining data.

As we near the end of the grant period, our focus will be on providing sustainability for the peer mentoring service. We will be reaching out by letter, email and telephone to stakeholders to actively propose partnerships in the coming weeks. We are currently in contract negotiations with one managed long term care plan and hope to come to an agreement on contract terms in the coming weeks. We will also be speaking with another provider who has expressed interest in moving forward with a partnership.

Following this report is a brief breakdown of the initial evaluation data, which are also indicated on the interim report of the project, which was completed in October. As part of our efforts to sustain the project, we will be sending this data to stakeholders throughout the state.
