Re: Information Request

We received your request for information on 9th April 2010. This will be dealt with in accordance with your rights of access under the Freedom of Information Act. To ensure we deal promptly and efficiently with your request we have listed the specific information you have requested below.

You request a copy, summary or extract of recorded information (if held by the PCT) in regards to the following request:

Regarding the rollout of Summary Care Record (SCR), please could you kindly provide the following information:

1.  Please could you confirm that it is indeed the policy of your PCT to force patients to opt out a second time if their opt out has already been recorded in their GP record for some while.

2.  Please could you confirm that it is indeed the policy of your PCT to only accept opt outs in writing, and only on the official CfH [Connecting for Health] form (in other words rejecting any opt out applications made on alternative opt out forms such as the one downloadable from The Big Opt Out website [www.thebigoptout.com]).

3.  Please could you send me ALL communication (letters, emails, faxes etc) to GPs that your PCT has made regarding how to manage patients who have already opted out (i.e. prior to the PIP [public information programme] in your area).

If this is incorrect, please advise us as soon as possible.

If charges apply, please be aware that the information will not be released until payment is cleared.

We shall send you a response to your request within 20 working days. If we encounter unavoidable delays (e.g. staff absence or sickness) we may ask you for an extension of the time limit to help us deal with your request and avoid disruption to the public service we provide.
