CABLE TELEVISION ADDENDUM Unit: ______
Owner will provide optional standard (basic) cable television service to residents at their request. The cable service will be from (Cable Provider) or a comparable provider. Resident(s) agree to pay $ per month for cable service.
Resident will pay for these optional cable services with the monthly rental payment for the apartment. Terms within the Cable Television Addendum can be altered by [the owner] providing residents a 30-day written notice. Residents are required to provide a 30-day written notice to cancel cable service. If cable has not been paid on or before the 3rd of each month, the cable service may be disconnected. To reinstate cable service, the monthly cable and any and all late charges plus a $25 reconnect fee must be paid by money order. Additional conditions are as follows:
1 Owner reserves the right to change the cable channels and services provided during the period of the lease as long as the change (1) reflects the cable company’s change in its standard cable TV service and (2) applies to all residents in the apartment complex.
2 Residents may obtain premium services (more than standard service) directly from the cable company. Residents will pay the cost of those premium services direct to the cable company.
3 Owner may, in addition to all rights and remedies available under the lease, terminate cable service in the situation of non-payment of cable service and/or lease amounts.
4 Owner does not provide a master system.
5 Owner will endeavor to cause the cable company to supply a quality signal. However, owner is not responsible for temporary outages and reception problems. If service is lost for more than one week due to faulty equipment or the fault of the cable company, residents will receive a prorated refund for that period in the form of a credit against future payments for cable services.
6 Residents are responsible for any cable equipment/box provided for use by the resident and agree to pay for any damages to the cable equipment/box. The charge for an unreturned or damaged cable box or remote is $50 each.
7 Cable services will be prorated at move-in and refundable at move-out if the apartment is vacant before the end of the month [with proper written notice].
