Release and Indemnity Agreement

I______, being the parent (or Guardian)(“the Undersigned”) of ______, (“the Player”), allow the Player to participate voluntarily in the Quakertown Connie Mack Youth Baseball League (“QCMYBL”) do hereby

or I ______, (“the Player”) being of legal age do hereby:

  1. Acknowledge that the Player’s participation in the QCMYBL, including summer, off-season training and other general athletic activities, will expose the Player to risk of minor or severe bodily injury up to and including death, and other harm, for which the Undersigned (or Player if of legal age) may suffer damages, including liability for medical expenses and other losses.
  2. Assume all risks and hazards incidental to the conduct of the QCMYBL activities including, but not limited to, practice equipment, travel to and from QCMYBL activities, and the game of baseball itself and all related physical training associated with baseball including the rendering of first aid emergency treatment.
  3. Acknowledge that QCMYBL requires that the Undersigned (or Player if of legal age) obtain and maintain medical insurance for the Player.
  4. Represent that the Player is physically fit to participate safely in the QCMYBL activities.
  5. Agree to comply with whatever requirements QCMYBL may impose concerning physical examinations of the Player, but acknowledge the Undersigned (or Player if of legal age) must decide upon the frequency of physical examinations to determine whether it is safe for the Player to participate in QCMYBL activities.
  6. Agree to provide QCMYBL any and all medical or other information relevant to the Player’s participation in QCMYBL activities.
  7. Agree to be financially responsible for the return of all athletic equipment issued to the Player.
  8. Agree to provide QCMYBL a certified birth certificate of the Player on registration day if requested.
  9. And in consideration of the QCMYBL allowing the Player to participate in the QCMYBL activities, release and covenant not to sue QCMYBL, and all QCMYBL’s officers, agents, organizers, sponsors, and coaches, for any claim, of any kind, in any way related to or as a result of the Player’s participation in any QCMYBL activity, (including without limitation, transportation provided in connection therewith) whether the injury results from the alleged negligence of any act of QCMYBL officers, agents, organizers, sponsors, umpires, or coaches excepting only injuries suffered as a result of an act of a QCMYBL officer, agent, organizer, sponsor, umpire, or coach which was intended to cause injury to the Player; and agree to indemnify QCMYBL and all QCMYBL officers, agents, organizers, sponsors, umpires, and coaches harmless from any and all claims, of any kind, made by the Player or any other person in any way arising out of the Players participation in QCMYBL activities, (including, without limitation, transportation provided in connection therewith) whether resulting from the alleged negligence of any other act of QCMYBL or any QCMYBL officers, agents, organizers, sponsors, umpires, or coaches, excepting only injuries suffered as a result of any act of a QCMYBL officer, agent, organizer, sponsor, umpire, or coach which was intended to cause injury to the Player or other injury which is the basis of the claim.
  10. The Release of Indemnity Agreement applies to all QCMYBL activities in which the Player participates during the current season, which includes any and all equipment that is loaned to the Player by QCMYBL.

Code of Conduct for Players and Parents

QCMYB has created this “Code of Conduct” for the express purpose of ensuring a safe and educational arena for our communities’ youth sports programs. By signing the registration form, we (as parents, coaches, and participants) agree to adhere to this code of conduct and understand the consequences of our actions.

** Please share and read this information with your child **

  • At no time during the course of a QCMYBL activity will a parent confront a coach for any reason, unless there is considered to be an emergency regarding the physical well being of a participant. In this event, the conversation must be done in a formal and quiet fashion
  • At no time during the course of a QCMYBL activity will a coach, parent, or participant use profanity.
  • At no time during the course of a QCMYBL activity will a coach, parent, or participant misuse or damage any equipment.
  • The player must be in full uniform, provided by QCMYBL, or the player will be suspended for the game.
  • At no time during the course of a QCMYBL activity will a parent involve themselves in a discussion of the rules or calls made by a game official.
  • At no time during the course of a QCMYBL activity will a coach, parent, or participant be involved in an abusive verbal or physical confrontation with another parent, coach, official, or participant.
  • At no time during the course of a QCMYBL activity will a coach, parent, or participant indulge in the use of Tobacco, Alcohol, or drugs. Persons that need to smoke must do so out of sight of the bench and participants.

Consequences for violating any of the above

  • Physical abuse violations will result in an immediate and permanent dismissal from ALL QCMYBL activities.
  • A 1st violation, other than for physical abuse, will result in an immediate suspension handed down by an officer and/or coach of QCMYBL. Violators will be suspended from all activities until the QCMYBL officers have reviewed the infraction.
  • A 2nd violation will result in the immediate dismissal from ALL QCMYBL activities for the remainder of the season. This 2nd violation will have no time limitations on the prior infraction. Infractions are cumulative.

All infractions must be submitted, in writing, to a QCMYBL officer within 1 week of the incident. At that time, the QCMYBL officers will review the infraction and determine any and all consequences. All decisions made by QCMYBL will be final.
