Purpose of Procedure

To provide a clear process for loaning equipment from the Short Term Loan Equipment Pool.

Background

  • A read only spreadsheet of Novita Short Term Loan Equipment is available. See Short Term Loan Equipment List.

Loan of this equipment is for periods up to 4 weeks only (except for power wheelchairs, post-op equipment and treadmills which can be up to 1 term).

  • This equipment has been purchased using Novita funds but is available for DSA client use following approval by the relevant ES senior clinician and subject to availability. Equipment Only registration for DSA clients is required before accessing this equipment.
  • Extensions to trial periods to be negotiated with the Equipment Stores Administration Officer. In some instances, the rationale for extension may need to be discussed with the relevant clinician from ES.
  • Other equipment may be available from suppliers. If required to accept liability for loss or damage to loaned equipment, permission and sign off is required from the Manager ES. If high cost equipment is to be transported, separate insurance may be necessary and can be arranged via ES TL / SAM.

Procedure

  1. Equipment Service (ES)& Regional Service (RS)cliniciansmay have priority for loans of the ES short term loan equipment items, depending on demands such as ES clinics and regional visits.
  2. ES short term loan equipment may be recalled at short notice by the Equipment Store Administration Officer at the request of ES or RS staff. (eg: for assessment and/or clinic purposes)
  3. Clinicians may borrow equipment as per Equipment Item Responsibilitiesfrom the Short Term Loan Equipment Pool.
  4. Check the Equipment Loans Database for availability of the desired equipment. Updated information regarding status of availability can be obtained by contacting the ES store administration officer (Ph: 8243 8302)
  5. To borrow equipment the clinician emails Short Term Loan Equipment() outlining the item of equipment requested, the barcode and preferred dates for borrowing.
  6. ES store staff will arrange delivery of the available item(s) to / from the relevant clinicians. Delivery to / fromEquipment Only clients will be arranged on an individual basis in consultation with ES store coordinator.
  1. Before sending the equipment,ES Store staff will ensure that:
  • The equipment is clean
  • The equipment is in working order and the batteries are charged
  • Any relevant instructions are provided with the equipment
  • The equipment is packaged safely and delivery organised.
  1. On receipt of the equipment, the clinician will sign the Short Term Loan Equipment Trial Agreement Form and return the signed copy to ES store administration officer (fax: 8243 8301).
  2. Before returning equipment the clinician will ensure that:
  • The equipment is clean or indicate to ES staff if specialised cleaning is required (eg: pressure cleaning)
  • For technology / communication equipment; that any personalised information has been backed up.
  • For technology / communication equipment; that any personalised information has been deleted from the equipment (seek support from ES staff if required).
