AspartoftheResidentialCarelicenserenewalprocess,licenseeswillberequiredtoreviewtheirrecordsanddocumentthereviewonchecklistspriortothelicensingstudyvisit.
Thelicenseewill review 20%ofthe currentrecordsin each area(personnel,children,shelterhomeparents,asapplicable).Ifyouarealicenseewithmultiplelicensedprogramswhicharebeingrenewedatonetimethis20%mustbetakenfromeachlicensedprogram.Ifstaffmembersareassignedtoworkinmultipleprogramsthey shouldbeconsideredanemployeeoftheprogramtheyworkatthemajorityofthetime.Forchildren's records,thereview should include20%ofchildrencurrentlyincare*whohavebeenadmittedsincethelastlicensewasissued.Thelicenseeshouldreviewaminimumof10and amaximumof25recordsofeachtype.(Note:Ifthetotalnumberofcurrentrecordsisfewerthan10,thelicenseeshouldreviewallrecords.)Inaddition,thelicenseewillreviewthreechildren'srecordsclosedwithinthelast24monthstoverifycompliancewithrequired dischargeinformation.ThelicenseewilldocumentthereviewonchecklistformsprovidedbytheDepartment.

1.Thelicenseewillfill-intheinformationlistedatthetopofeachchecklistform.
2.Foreachfilereviewed,thelicenseewillindicatethat therequired information is included inthefilebymarkingacheckbesidetheboxindicatingtheEECregulatoryrequirement.Someboxesallowdocumentationofmorethanonerequirement.Lineshave beenprovidedforeachcheckmarkneededinthoseboxes.
3.Thelicenseemaymakeadditionalcopiesoftheblankchecklistifmoreareneededtodocumentreviewofthenecessaryrecords.
4.Thelicenseewillretaintheoriginalofthecompletedchecklistformsandmaintainthemintheirrecordsuntilthenextlicensingstudy.Licensorswillreviewthesechecklistsduringthe licensingstudy.
*Iftheprogramserveschildrentaking antipsychoticmedication, thelicenseeshould includeat leastonesuchrecordinitsreview.
