Project Details (20 points)

·  Provide a clear explanation of the need (i.e., problem or gap) that currently exists and how the proposed innovative project will contribute to a solution and facilitate student success.
·  Provide relevant data, literature, and documented effective practices related to the proposed project.

Success Indicators and Evaluation (20 points)

·  List the goals for the project and explain how you plan to accomplish them.
·  Describe assessment tools (i.e., testing instruments, surveys, observations, and/or questionnaires) that will be used for the project.

Governing Board Outcomes and Strategic Commitments (10 points)

·  Explain how the project aligns to one or more MCCCD Strategic Commitments, and MCCCD Governing Board Outcomes and Metrics
·  List specific references to MCCCD outcomes, metrics, and strategic commitments:
o  University Transfer Education and General Education
o  Workforce and Economic Development
o  Developmental Education
o  Community Development and Civic and Global Engagement

Personnel (5 points)

·  Describe the roles, responsibilities, and qualifications for personnel required to complete the project.

Dissemination Plan (5 points)

·  Explain how the results of the project will be disseminated both internally and externally (e.g., department meetings, national conferences, MCCCD conferences, discipline specific conferences).

Sustainability (5 points)

·  Explain how the project will be sustained following the funding year.
·  Describe the extent and means by which the project will be integrated into the college/department within a defined time frame.

Type the amounts for Faculty compensation and/or miscellaneous expenses that will be paid for by this grant. Wages must include the faculty rate of $28.00 per hour and 20% for statutory benefits.
