PRIMARY JOB FUNCTIONS AND PERFORMANCE EXPECTATIONS

JOB CLASSIFICATION: Residential Instructor I, Parkland Place

Responsible to: Coordinator Parkland Place

Supervises: N/A

I. Assists in developing client=s Individual Plan of Care (IPC).

20%

a. Actively participates in assessing client=s strengths, needs, and skill deficiencies, with an emphasis on social / emotion issues.

b. Reviews intake material including: Medical History, Psychological History, Education / Vocational History, and other information as appropriate.

c. Actively participates in ongoing team staffing designed to develop / revise the individual plan of care.

d. Actively participates in establishing specific goals and observable / measurable objectives for needed client skills development.

II. Implements Individual Plan of Care (IPC) through the provision of direct

30% services to the client.

a. Constructs BLS lesson plans based upon knowledge of the client=s abilities / deficiencies.

b. Conducts and supervises BLS training and educational activities in accordance with lesson plans, IPCs and program schedule.

c. Appropriately handles self-injurious, aggressive and disruptive behavior in accordance with program procedures and expectations.

d. Effectively utilizes appropriate resources necessary to carry out client program.

e. Accurately administers medications and PRNs as prescribed or instructed.

f. Monitors and treats special health conditions of the resident(s), as per instructions.

g. Responds to emergency situations consistent with Glenwood, Inc. policies and procedures or makes independent decisions consistent with safety and well being of the residents.

h. Performs routine counseling when necessary and advises supervisor of the need for more expert counseling.

i. Provides supervision consistent with individual treatment plans on off-campus activities.

III. Provides support services for the home and the residents.

20%

a. Participates in the cleanliness of the home consistent with Glenwood, Inc., DMH, and Health Department standards.

b. Responsible for assisting in the maintenance and general operating condition of the vehicle in accordance with policy.

c. Participates in maintaining the home, vehicle and yard in an acceptable manner as assigned. Informs Coordinator, Parkland Place of repairs needed.

d. Makes minor repairs and other general routine maintenance tasks as assigned or implied.

e. Monitors supplies (food, paper products, cleaning products, etc.) And informs Coordinator, Parkland Place of needs.

f. Visually inspects the home to ensure that resident work tasks have been completed or performed in an acceptable manner.

IV. Transports clients in accordance with schedule.

a. Drives the van to transport clients as per scheduled and unscheduled activities.

b. Drives the van with no at fault= accidents.

c. Ensures that all client safety and conduct procedures are adhered to.

d. Drives van in accordance with all state and local traffic laws and regulations.

e. Escorts clients on shopping trips or other activities.

f. Assists and transports clients to scheduled appointments in the community.

V. Completes required verbal and written administrative and client related

20% documentation.

a. Accurately records medications dispensed and changes in prescriptions in accordance with program procedures.

b. Records appropriately client behavior, progress and problems consistent with instructions and procedures.

c. Ensures that receipts and other documentation as to client and program expenditures are properly recorded and turned in to home manager.

d. Ensures that all filing is done daily so that clients= charts remain current.

e. Completes all reporting documents as assigned.

performance improvement team functions with data analysis, root cause analysis, action planning or evaluation.

b. Adheres to scheduled work hours.

c Reports to work each day as scheduled, prepared to complete job responsibilities.

d. Approaches job with a positive attitude, keeping client needs as priority.

e. Appropriately applies for and utilizes leave, based on agency policy.

f. Attends all required staff meetings, treatment team meetings, agency conferences, and in-services.

g. Supports agency policy and procedures.

h. Exhibits respect for coworkers and supervisors.

i. Communicates directly to the person involved in a professional manner to resolve conflicts as they occur.

j. Completes all job responsibilities within a given time frame.

k. Attends at least one conference annually related to client care.

l. Obtains and maintains certification in CPR, First Aid, PMAB, Diversity Training, Defense Driver Training and other certifications as needed.

m. Performs tasks / assignments according to instructions.

Classification: Salaried - Nonexempt

Minimum Qualifications: Minimum of 21 years of age. Diploma or equivalent required. Experience in residential care preferred. Current community CPR and First Aid, Red Cross certification. Current Alabama Drivers License and must be insurable through Glenwood, Inc. contracting agencies.
