These scholarships are intended to recognize outstanding athletes at

Presentation of Mary Academy.

Seniors who wish to apply for the Stephen Stankus Athletic Scholarship should fill out this application form.

Applicants will be evaluated on the basis of their perseverance, dedication, sportsmanship,

athletic achievement(s) and leadership.

To be considered for a scholarship, a student must meet the following criteria:

• Athletic Participation. Applicant must be a student in good standing at Presentation of Mary Academy and who has participated in at least one varsity or junior varsity athletic season during the school year .

• Application Form. Student must complete and submit attached application.

One recommendation checklist from a coach must accompany the completed application. The checklist should be submitted in a sealed envelope. (Recommendation checklist form attached.)

• Submission. The completed application form, including this cover sheet and one sealed recommendation checklist, must be delivered to the Guidance Office, no later than May 6, 2011. Incomplete applications will not be considered. Applications turned in after the deadline will not be considered.

Application Deadline is May 6, 2011

Scholarship winners will be announced at Graduation for seniors and end of the year Recognition Program for underclassmen.

Each award will be credited to the students tuition account for the next school year, except graduating seniors, who will receive a check made payable to the student.

(The returning PMA student must complete the entire school year at PMA.)
