As you are reading this letter on our website, you are aware of the ease and convenience of our electronic registration process. Thank you for taking advantage of this and please take a look around the website and familiarize yourself with the various features. All of our new patient forms are available for download and printing, simplifying the registration process once you come in for your appointment. Please note that if you have not completed the required paperwork prior to your visit and you do not arrive 30 minutes early, or you arrive without your insurance card, ID, or payment, your appointment will be rescheduled. We do our best to stay on time so we ask that you do the same please.

Please do not wear cologne or perfume to this appointment.

We are located in the Medical Office Building at Augusta Health off of Goose Creek Road in Fishersville. Turn right into the Augusta Health grounds. At 4-way-stop, turn right. At 3-way-stop, go straight. At second 4-way-stop, turn left and park in parking lot C. You will see the MOB building in front of you. We are located on the 2nd floor in Suite 210, which is the only office to the left of the elevator as you get off.

If you would like us to file your insurance claim, we will need a copy of your insurance I.D. card and a picture I.D. or driver’s license when you arrive. The appropriate I.D. is required to prevent insurance and financial fraud.

We recommend that you check with your insurance carrier to insure that we are in their network and to determine if a referral is required. As required by your insurance, all insurance co-pays and deductibles are due upon check in. Medicare co-insurance (20% of the allowable fee) will be collected at the time of service if there is not a supplemental insurance. We accept credit cards (MasterCard & Visa), debit cards and checks only.

A broken appointment is a loss to everyone. PLEASE inform us ONE day in advance if you are unable to keep your appointment. Appointment reminders are a courtesy we attempt to provide. Our office reserves the right to charge a $25.00 fee for missed appointments.
